摘要
目的观察单应用关节镜清理术和关节镜清理联合关节腔内注射玻璃酸钠两者对膝骨性关节炎的疗效。方法 2003年6月～2008年l2月共采用单纯关节镜下关节清理术和关节镜下关节清理术加腔内注射玻璃酸钠两种方法治疗骨性关节炎患者共558例,其中单纯行关节镜清理术251例,关节镜清理术联合玻璃酸钠治疗307例。结果所有患者均经过临床随访6～70个月,平均36.5个月,单纯关节镜下关节清理术优良率为79.2%,而关节清理术加腔内注射玻璃酸钠治疗后的优良率高达93.6%;按Lequence标准膝关节评分,两组病例治疗前后差异均有显著性,但是关节清理术加腔内注射玻璃酸钠治疗24个月后疗效明显优于单纯关节清理术(P<0.05)。结论应用关节镜清理配合关节腔内注射玻璃酸钠治疗骨性关节炎,明显提高了治疗效果,值得推广。
【Objective】 To observe the clinical effect of arthroscopic debridement alone and arthroscopic debridement combined with injection of sodium hyaluronate in treatment of osteoarthritis of knees,and to compare the effects of two treatments.【Methods】 From Jun.2003 ～ Dec.2008 558 cases of osteoarthritis of knees,251 cases used arthroscopic debridement alone,other 307 cases used arthroscopic debridement combined with injection of sodium hyaluronate were analyzed.【Results】 The two groups were followed-up for 6～70 months with an average of 36.5 months.the excellent and good rate of the knee joint function was 93.6% in arthroscopic debridement combined with injection of sodium hyaluronate group,but it was 79.2% in arthroscopic debridement group;According to the Lequesne index scores,there was a significant difference（P 0.05） before and after treatment in two groups,but in 24 month after treatment,the clinical effect of arthroscopic debridement combined with injection of sodium hyaluronate in treatment of osteoarthritis of knees was better than arthroscopic debridement alone.【Conclusion】 Application of arthroscopic debridement combined with injection of sodium hyaluronate in treatment of osteoarthritis of knees can clearly raise clinical effect,it is worthy to be popularized.
