摘要
目的探讨血清载脂蛋白浓度变化与糖尿病肾病(DN)进展之间的关系。方法将837例2型糖尿病患者分为单纯糖尿病(SDM)组524例、早期糖尿病肾病(EDN)组208例和临床糖尿病肾病(CDN)组105例,分别测定血清载脂蛋白水平,并与正常对照组比较。结果 SDM组血清载脂蛋白浓度(APOA1、APOA1/B100)与正常对照组比较差异无统计学意义。EDN、CDN组血清APOA1、APOA1/B100浓度明显低于正常对照组和SDM组(P<0.01)。SDM组血清载脂蛋白B10(0APOB100)浓度明显高于正常对照组,CDN组APOB100水平明显高于SDM、EDN组(P<0.01)。尿白蛋白排泄率(UAER)与血清APOA1、APOA1/B100浓度呈直线负相关(r=-0.1765,P=0.011;r=-0.2018,P=0.0034),与APOB100浓度呈直线正相关(r=0.2216,P=0.0013)。结论 2型糖尿病患者随着尿UAER逐渐升高,血清APOA1、APOA1/B100逐渐下降,APOB100逐渐升高,提示血清载脂蛋白水平与DN进展有关。
Objective To investigate the relationship between apolipoprotein levels and evolution of diabetic nephropathy (DN). Methods 837 patients with type 2 diabetes mellitus (T2DM) were divided into 3 groups according to the Mogensen criterion of DN: 524 simple diabetes mellitus (SDM), 208 with microalbuminuria (EDN) and 105 with clinical albuminuria (CDN). Apolipoprotein was tested in all of them, and compared with Control group. Results Apolipoprotein A1 (APOA1)and apolipoprotein A1/apolipoprotein B100 (APOA1/B100) levels were similar between group SDN and control group (P〉0.05). But in the group EDN and CDN, APOA1 and APOA1/B100 were significantly lower as compared with the former two groups (P〈0.01). In the group SDM, apolipoprotein B 100 (APOB100)was significantly higher than that in control group, but in the group CDN, APOB100 was significantly higher than that in the group SDM anti group EDN (P〈0.01). There was a negative correlation between APOA1 , APOA1/B00 concentration and UAER (r=-0.176 5,P=0.01 1 ;r=-0.201 8,P=0.003 4 ), but there was a positive correlation between APOB100 concentration and UAER (r=0.221 6, P=0.001 3). Conclusion With the UAER raising, the levels of APOA1 and APOA1/B100 gradually declined and APOB100 gradually rose in patients with type 2 diabetes. Serum apolipoprotein level may be related to the evolution of DN.
