摘要
目的应用应变率成像(SRI)技术对比冠心病和高血压病患者的左室心肌功能,探讨SRI评价左室心肌功能的临床价值。方法 A组为健康体检人群(n=29);B组冠心病患者(n=40),其中B1组为心绞痛患者19例,B2组为心肌梗死患者21例;C组原发性高血压病患者(n=52),据动态血压分C1组(杓型组)24例和C2组(非杓型组)28例;D组冠心病合并原发性高血压患者(n=20)。测定各组左室各壁的峰值收缩期应变率(PSSR),计算平均PSSR;统计各组心室壁出现收缩后缩短(PSS)的节段数。结果与A组相比,B1、B2、C1、C2及D组mPSSR均明显降低(P<0.05);与B1组、D组相比,B2组mPSSR明显降低(P<0.05);与C1组相比,D组、C2组mPSSR均明显降低(P<0.05);与A、C组相比,B、D组PSS的检出率明显增高(P<0.05),而A组比C组、B组、D组间PSS检出率比较差异无统计学意义(P>0.05)。结论 SRI技术为定量、准确评价冠心病和高血压患者左室心肌功能提供了新的方法。
Objective To explore the clinical value of strain rate imaging(SRI) technotogy m comparing the left ventricular cardiac function in patients with coronary heart disease and hypertension. Methods Twenty-nine healthy volunteers were treated as control group (group A), forty patients with coronary heart disease (group B )were divided into two subgroups: group B1 ( angina pectoris, n = 19 ) and h myocardium infarction ( group B2, n = 21 ), fifty-two patients with hypertension ( group C ) were divided into two groups according to ambulatory Blood pressure profiles : group C 1 (dippers) and group C2 ( nondippers), twenty patients with essential hypertension and coronary heart disease( group D). The the peak systolic strain rate (PSSR)during systole were measured in all patients, the mean PSSR (mPSSR)were calculated. The numbers of post systolic deformation(PSS) segments were recorded and analyzed in the left ventricular walls. Results mPSSR of group B1, B2, C1, C2 and D were significantly lower than that of group A, respectively ( P 〈 0.05 ) ; mPSSR of group B2 was significantly lower than those of groups B1 and D ; mPSSR of group D and C2 was significantly lower than that of group C1, respectively ( P 〈 0.05 ). The presentation of PSS. in the left ventricle of groups B and D were significantly lower than those of group A and C ( P 〈 0.05 ), while the differences between group A and C as well as group B and D were not significant(P 〉 0. 05). Conclusion SRI provides us a new and accurate method, to evaluate the left ventricular systolic function quantitatively in patients with coronary heart disease and hypertension.
