摘要
目的观察生长抑素联合皮硝外敷对重症急性胰腺炎（SAP）患者免疫调节因子的影响及其临床疗效。方法 2006年1月至2009年6月共纳入的39例SAP患者按随机数字表分组,其中研究组20例,为生长抑素联合皮硝外敷组;对照组19例,为生长抑素组。于治疗前后检测血清免疫调节因子[白细胞介素（IL）-1、IL-6和肿瘤坏死因子（TNF）-α]的动态水平,同时记录病死率和并发症发生率。另对20例健康自愿献血者（空白对照组）作IL-1、IL-6和TNF-α的正常值检测。结果①对照组与治疗组治疗前的IL-1、IL-6和TNF-α明显高于空白对照组（P〈0.05）,但两组间差异无统计学意义;②研究组治疗后8 d检测显示,血清中细胞因子IL-1、IL-6和TNF-α的浓度下降速度较对照组明显（P〈0.05）。③两组间器官功能不全发生率和局部并发症发生率、病死率差异有统计学意义（P〈0.05）。结论早期短程使用生长抑素联合皮硝外敷能改善SAP患者受损的细胞免疫功能,改善病情,提高临床疗效。
Objective To observe the clinical significance and changes of immune regulative factors in patients with severe acute pancreatitis（SAP）after the administration of somatostatin combined with mirabilite exteral applying.Methods Thirty-nine patients with SAP were randomly devided into experimental group and control group.Administration of somatostatin combined with mirabilite exteral applying was given to experimental group,administration of somatostatin was given to control group.To detect dynamic level of serum immune regulatory factors（IL-1,IL-6 and TNF-α） and to survey the change of mortality and complications rate in the patients after the treatment.Another group of 20 healthy volunteers were recruited for testing the normal levels of serum immune regulatory factors（IL-1,IL-6 and TNF-α）.Results ①The levels of serum immune regulatory factors（IL-1,IL-6 and TNF-α） before treatment in both groups were significantly higher than those in the healthy subjects（P0.05）,but there was no statistical difference between the two groups.②Indexes of serum immune regulatory factors（IL-1,IL-6 and TNF-α）were more significantly decreased in experimental group than those in control group on the 8 days after the treatment（P0.05）.③The occurrences of organic dysfunctions,local complications and mortality in both groups were statistically different.Conclusion The short course administration of somatostatin combined with mirabilite exteral applying continuously at early stage can up-regulate the cell immune function and improve the conditions of the patients with SAP,and have better clinical results.
