摘要 目的观察在数字血管减影机(DSA)介导下应用医用臭氧治疗腰椎间盘突出症的临床疗效。方法腰椎间盘突出症52例,根据CT和/或MRI表现将椎间盘突出分为膨出、突出2种类型,比较不同类型椎间盘突出的治疗结果;在DSA介导下,均经皮腰椎间盘内注射浓度为40μg/ml医用臭氧8~12ml,退针至椎间孔附近时,向椎旁组织内神经根旁注入臭氧10ml及复方倍它米松1ml+2%利多卡因0.5ml+维生素B12注射液1ml(0.5mg/ml)的混合液;并按照纤维环是否破裂分组研究纤维环破裂与疗效的关系。结果 52例腰椎间盘突出症患者行臭氧注射治疗后6个月复查45例有效,总有效率为86.54%,其中12例椎间盘膨出10例有效(83.33%),40例椎间盘突出35例有效(87.50%)。28例纤维环破裂者26例(92.93%)有效,24例纤维环完整者19例(79.16%)有效;统计学差异均有显著性。所有病倒均无任何并发症。结论运用臭氧治疗腰椎间盘突出症方法疗效确切,相对于膨出,突出类型疗效高,但无统计学意义;相比纤维环完整者,纤维环破裂疗效较好。 Objective To evaluate the therapeutic effect and indication for the treatment of lumbar disc herniation by using intradiscal and paraspinal injection with O2-O3 mixture. Methods Fifty-two patients were selected and divided into two groups by their category of the lumbar disc herniation which were verified by CT or MR imaging. For each herniated disc treatment, 8 - 12 ml O2-O3 mixture at 40 μg/ml was injected into intradiscal space, and 10 ml O2-O3 mixture was also injected into the paravertebral space when the needle withdrawned near the nerve root of the intervertebral forame, then the mixture of compound betamethason (1 ml), lidocaine (0.5 ml) and vitamin B12 (0.5 mg) was injected. They were also divided into two groups with or without fibrous annulus, the correlation between fibrous annulus broken and therapeutic effect was studied. Results The patients were followed up for 6 months after treatment The total efficacy was 86.54% (45/52). Among them, 10 cases (83.33%) in 12 bulging patients and 35 cases (87.5%) in 40 patients protrusion had excellent recovery; there was no significant difference in therapeutic effect existed between bulging patients and protrusion patients. 26 eases (92.93%) in 28 patients with broken fibrous annulus and 19 cases (79.16%) in 24 patients with intact fibrous annulus had excellent recovery; there was significant difference in therapeutic effect between broken fibrous annulus group and intact fibrous annulus group. No serious complication occurred. Conclusion The therapeutic method developed by injecting O2-O3 mixture in lumbar intradiscal and paraspinal space is a safe and effective method for the treatment of the lumbar disc herniation. The therapeutic effect is fine in broken fibrous annulus group and there is no significant difference between bulging group and protrusion group.
