重复经颅磁刺激合并药物治疗广泛性焦虑症早期疗效的初步研究 被引量:13

Preliminary study on the early-stage efficacy of repetitive transcranial magnetic stimulation(rTMS)in combination with medication in the treatment of generalized anxiety disorder(GAD)

摘要 目的 探讨重复经颅磁刺激(rTMS)联合药物对广泛性焦虑症(GAD)早期治疗阶段的效果.方法 将首诊的GAD患者随机分成两组,研究组采用低频对右侧前额叶背外侧皮质进行rTMS联合帕罗西汀+苯二氮类药物治疗,对照组采用单纯帕罗西汀+苯二氮类药物治疗.采用汉密尔顿焦虑量表(HAMA)评价疗效、副反应量表(TESS)评价安全性.结果 研究组完成34例,对照组完成32例,治疗1周后两组HAMA均分比较无显著差异(P>0.05);治疗2周后研究组HAMA均分显著低于对照组(P<0.05),研究组帕罗西汀和苯二氮类药物治疗剂量均显著小于对照组(P<0.05);研究组副反应发生率小于对照组.结论 重复经颅磁刺激合并药物对GAD早期阶段的治疗比单纯药物治疗起效快,疗效佳,可减少抗焦虑药的用量,是一种治疗GAD的有效手段. Objective To investigate the early--stage efficacy of repetitive transcranial magnetic stimulation(rTMS) in addition to medication in the treatment of generalized anxiety disorder(GAD). Methods GAD patients were randomly divided into two study groups. Treatment group of 34 patients received treatment with low rate TMS on the right dorsolateral prefrontal cortex together with Paroxetine and Benzodiazepine,control group of 32 patients with Paroxetine and Benzodiazepine only. The Hamilton Anxiety Scale (HAMA) and the Treatment Emergent Symptom Scale (TESS) were utilized in evaluating of curative efficacy and safety. ResuLts There was no significant difference between the average HAMA points in either of two groups before and one week after the treatment (P 〈0.05). However, there was significant difference between the average HAMA points before and two weeks after the treatment (P〈0.05). Therapy doses of Paroxetine and Benzodiazepine for the treatment group were all lower than that in the control group (P 〈 0.0S) , and the incidence rate of side efficacy in the treatment group was lower than that of the control group. Conclusions In the treatment of GAD,rTMS in combination with medication provides faster and better therapeutic efficacy than the drug only treatment ; moreover,it can also reduce the dose of anti-anxiety drugs and is thus an effective supplementary means in the treatment of OAD.
