摘要
目的探讨腹部外科感染性疾病病原菌的构成比和药物敏感率的变化。方法采用美国BD公司生产的6B和7D两种增菌瓶采集标本和培养细菌,并用该公司生产的生化板和药敏板对腹部外科常见的感染性疾病患者共269例,进行细菌学和药物敏感度的检测。结果269例标本,其中206例标本细菌培养阳性,阳性率为766%。185例标本检出需氧菌共271株,检出率为688%,以埃希大肠杆菌为主(112株)占需氧菌总数的413%,其次是变形杆菌(144%),肠球菌(63%);141例标本厌氧菌培养阳性,共检出296株菌,总检出率为524%,平均每例21株,以脆弱类杆菌最多共129株,占厌氧菌的436%,其中又以脆弱拟杆菌为主(63株),其次是吉氏拟杆菌(28株)。所有阳性标本中单纯需氧菌和单纯厌氧培养阳性率分别为316%和102%,而混合感染阳性率为583%。革兰阴性(简称G阴性)菌占主要地位,为需氧菌的731%和厌氧菌的682%。需氧菌对三代头孢菌素敏感率在80%~100%,而对氨基甙类敏感率在327%~500%间,厌氧菌对甲硝唑敏感率达946%,氯洁霉素为85.0%。结论所有菌种都是人体正常菌种,条件致病?
Objective To
investigate the infectious rates of pathogenic bacteria and their drug sensitivity in common
infection diseases of abdominal surgery in local hospitals. MethodsBZThe specimen from 269
cases with the common infectious diseases of abdominal surgery were collected and cultivated
on two types of enrichment bottles, 6B and 7D, which were made by the BD company in
America. At the same time, we used the biochemical and drug sensitive plates to identify the
types of bacteria and their drug sensitivities. HZ Results Among 269 specimen,cultures were
positive in 206 (766%). 271 strains of aerobes were found in 185 specimen, with an incidence of
688%, among which E.coli(112 strains) came first (413%), then proteus (144%) and Enterococcus
(63%). 269 strains of anaerobes were found in 141 specimen, with an incidence of 524% and 2.1
strains per case, the most common type was B.fragilis (63 strains), followed by B.distasonis
(28 strains). In the positive cases, unitary aerobic and unitary anaerobic infection rates were
only 316% and 102% respectively while mixed infection rate was 583%. Most of the pathogens
were gramnegative, 711% in aerobes and 682% in anaerobes. The drug sensitive rate of
aerobes to the 3rd generation cepholosporine were between 80% and 100% and between 327%
and 50% aminoglycocide. 946% of anaerobes were sensitive to metronidazole and 85% to
Clindamycin. HZConclusions All pathogenic bacteria were of normal bacteria in human body.
Infections complicated by abdominal surgery were characterized by opportunistic and
endogenous pathogens. The most common types were mixed infections of aerobic and
anaerobic bacteria. There were more anaerobic infections and most of pathogenic bacteria were
gramnegative, but drug resistant strains were relatively less in Kumming area than in other
