摘要
目的:分析高血压性脑出血患者不同时间内应用不同剂量甘露醇对脑出血血肿增大及患者肾功能的影响。方法:选择发病6h内,经CT证实血肿20～30mL的基底节区脑出血患者109例,分为2组:Ⅰ组54例,在12h内用甘露醇,其中26例(A组)应用125mL,28例(B组)应用250mL;Ⅱ组55例,12h后应用甘露醇,其中27例(C组)应用125mL,28例(D组)应用250mL。发病24h后复查CT,1周后复查肾功能,比较不同时间段应用不同剂量甘露醇对血肿扩大及肾功能的影响。结果:A组有8例患者血肿扩大,B组12例,C组2例,D组6例,即不同时间应用甘露醇血肿扩大比较,差异有统计学意义(P<0.05),相同时间应用不同剂量甘露醇血肿增大比较无显著性差异(P>0.05)。但应用小剂量甘露醇的患者肾功能明显改善。结论:发病12h内应用甘露醇增加血肿增大率,使病情加重;同时间段内给予125mL或250mL甘露醇对血肿扩大无明显影响,但小剂量甘露醇可减少肾功能损害。
OBJECTIVE：To analyze the influence of application time and dosage of mannitol on enlargement of hematoma and renal function in patients with hypertensive cerebral hemorrhage.METHODS：109 basal ganglia cerebral hemorrhage patients with 20-30 mL of hematoma volume determined by cranial CT within 6 h after the onset were randomly divided into 2 groups.54 cases in groupⅠ received mannitol within 12 h,among which 26 cases in group A received 125 mL mannitol,28 cases in group B 250 mL mannitol.55 cases in groupⅡ received mannitol after 12 h,among which 27 cases in group C received 125 mL mannitol and 28 cases in group D 250 mL mannitol.Cranial CT was redone at 24 h and renal function was checked after 1 week.Effects of different dose and different application time of mannitol on enlargement of hematoma and renal function were compared among those groups.RESULTS：Enlargement of hematoma occurred in 8 cases of group A,12 cases of group B,2 cases of group C and 6 cases of group D.There was significant difference in enlargement of hematoma between group Ⅰand groupⅡ（P0.05）.No significant difference was found in group A vs.group B and group C vs.group D（P0.05）.The renal function of patients treated with small-dose of mannitol was improved significantly.CONCLUSION：Application of mannitol within 12 h after the onset increases the incidence of early enlargement of hematoma and exacerbates the patient＇s condition;no significant difference in enlargement of hematoma has been found in patients treated with different dose of mannitol at the same time,but small-dose of mannitol could decrease the harm to renal function.
