摘要
目的观察灵术颗粒治疗多囊卵巢综合征(PCOS)患者的临床疗效,探讨灵术颗粒对PCOS患者月经周期、内分泌及脂代谢的影响。方法将符合纳入标准的患者62例采用随机数字表法分为两组各31例,试验组选用灵术颗粒治疗,对照组选用二甲双胍治疗,两组用药时间均为3个月经周期,比较用药前后体重指数(BMI)、腰臀比(WHR)、空腹血糖(FBS)、空腹胰岛素(FIN)、胰岛素敏感指数(ISI)、血脂及性激素的变化以及基础体温(BBT)双相周期率的情况,并进行中医证候评分。结果灵术颗粒对性激素、脂代谢[除低密度脂蛋白胆固醇(LDL-C)、甘油三酯(TG)外]影响均优于二甲双胍,差异有统计学意义(P<0.05)。灵术颗粒比二甲双胍能更好地降低肥胖型PCOS患者的BMI及WHR(P<0.05),两组患者降低胰岛素抵抗疗效相仿(P>0.05)。灵术颗粒治疗期间获得BBT双相周期率优于二甲双胍(P<0.05)。结论灵术颗粒可调整PCOS患者性激素水平,改善胰岛素抵抗,降低肥胖患者BMI及WHR,改善脂代谢异常,获得更多BBT双相周期,降低中医证候评分,且疗效优于二甲双胍。
Objective To observe the effect of Lingzhu Granules （Granule of Herba epimedii and Rhizoma Atractylodis Macrocephalae） on the menstrual cycle, endocrine and lipid metabolism of the patients with polycystic ovary syndrome （PCOS）. Methods With the random controlled method, the 62 PCOS patients enrolled were equally divided into two groups, the experimental group was treated with Lingzhu Granules and the control group with Metformin. Both groups were treated for three menstrual cycles. The changes in body mass index （BMI）, waist hip ratio （WHR）, serum follicle stimulating hormone （FSH）, luteinizing hormone （LH）, testosterone （T）, estradiol （E2）, fasting blood sugar （FBS）, fasting insulin （FINs）, insulin sensitive index （ISI）, blood lipid, and biphasic basal body temperature （BBT） periodicity were observed. Results The effect of Lingzhu Granules on the sex hormone and lipid metabolism （except for LDL-C and TG） was better than that of Metformin with significant difference （P〈0.05）. Lingzhu Granules could decrease the BMI and WHR of PCOS patients, better than Metformin （P〈0.05）. The lowering of insulin resistance in both groups was similar in effect （P〉0. 05）. A better biphasic BBT period was achieved in the Lingzhu Granules group during treatment, superior to that in the Metformin group （P〈0.05）. Conclusion Lingzhu Granules is effective in adjusting sex hormone, lowering BMI and WHR of the obese patients, improving abnormal lipid metabolism, acquiring more biphasic BBT period, and lowering the TCM syndrome scores of PCOS patients, with a better effect than Metformin.
