摘要
目的探讨腺苷脱氨酶(ADA)在糖尿病肾病患者血清中的变化及其临床意义。方法对98例2型糖尿病患者(无蛋白尿组30例、微量蛋白尿组35例、大量蛋白尿组33例)和30例健康体检者ADA、胱抑素C(CysC)、白介素6(IL-6)以及C反应蛋白(CRP)进行测定,并对比分析。结果各组糖尿病患者与对照组的ADA、CysC、CRP和IL-6含量差异均有统计学意义(P<0.01);微量蛋白尿组的ADA、CysC、CRP和IL-6水平明显高于无蛋白尿组(P<0.05);大量蛋白尿组的ADA、CysC、CRP和IL-6水平明显高于微量蛋白尿组(P<0.05)。ADA分别与CysC、CRP、IL-6成正相关。结论早期联合检测血清ADA及CRP、CysC、IL-6水平变化可以作为评估糖尿病肾病病情和预后的参考指标。
Objective To investigate the changes of serum adenosine deaminase level in patients with diabetic nephropathy.Methods Levels of serum ADA,CysC,CRP and IL-6 from 98 DM2 patients and 30 normal control subjects were measured.The diabetic patients were divied into three groups: GroupA,without albuminuria(urine albumin excretion rate UAER30 mg/d,n=30).Group B,micro-albuminuria(UAER300 mg/d,n=35).Group C,macro-albuminuria(UAER300 mg/d,n=33).Results Serum levels of ADA,CysC,CRP and IL-6 were significantly higher in all the patients with type 2 diabetes than those in controls(P0.01).Serum levels of ADA,CysC,CRP and IL-6 were significantly higher in DM patients with micro-albuminuria than those without albuminuria(P0.05).Serum levels of ADA,CysC,CRP and IL-6 were significantly higher in diabetic patients with macro-albuminuria than those with micro albuminuria(P0.05).ADA levels were positively correlated with CysC,CRP and IL-6(P0.05).Conclusion Serum ADA may play an important role in the occurrence and development of diabetic nephropath.Determination of serum ADA level might be helpful for early detection of diabetic nephropathy and monitoring the progress of the disease process.
