Effect of hormone therapy on ultrasonographic classification of female mammary gland

摘要 目的:分析激素治疗对女性乳腺超声分型的影响,探讨激素治疗与乳腺恶性病变的相关性。方法:激素治疗组196例,年龄40—70岁;非激素治疗组1307例,年龄16~90岁,其中经病理证实为恶性病变61例,1例为双侧病变;根据声图像显示的乳导管、纤维结缔组织及脂肪组织的比例将乳腺实质分为腺体型、腺纤维型、脂肪型,以单侧乳腺为统计单位;比较激素治疗组和非激素治疗组及乳腺恶性病变组乳腺超声分型随年龄变化的趋势。结果:非激素治疗组,腺体型乳腺的比例随年龄的增长下降,脂肪型的比例随年龄增长上升;腺纤维型乳腺在40~49岁组达到高峰,持续到50~59岁组,60岁以后比例下降。激素治疗组,腺纤维型乳腺的比例高于非激素治疗组,随年龄的变化趋势与非激素治疗组不同,几乎呈直线下降趋势;脂肪型乳腺的变化趋势与非激素治疗组相同。恶性病变组中腺纤维型乳腺随年龄变化的趋势及脂肪型的比例均与一般人群相似,而与激素治疗组明显不同。结论:激素治疗在一定程度上延缓了乳腺的退化,使乳腺发生恶性病变的危险性相对降低。 Objective : To analyze the effect of hormone therapy (HT) on uhrasonographie classification of female mammary gland, explore the correlation between HT and malignant breast lesions. Methods: HT group included 196 cases aged 40 -70 years old; non -HT group included 1 307 cases aged 16 -90 years old, and 61 cases were confirmed as malignant lesions by pathological examination, 1 case with bilateral lesions. According to the proportions of glandular duct, fibrous connective tissues and fatty tissues indicated by ultrasonic ima- ges, mammary gland were divided into glandular type, glandular- fibrous type and fatty type, unilateral mammary gland was regarded as a statistical unit ; the change trends of ultrasonographic classification of female mammary gland with age were compared among HT group, non - HT group and malignant lesions group. Results : In non - HT group, the proportion of glandular type decreased with age ; the proportion of fatty type increased with age ; the proportion of glandular - fibrous type achieved a peak in 40 - 49 years group, sustained to 50 - 59 years, and decreased after 60 years. In HT group, the proportion of glandular - fibrous type was significantly higher than that in non - HT group, showing a linear decrease tendency ; the changing tendency of fatty type was the same as non - HT group. In malignant lesions group, the changing tendency of glandular - fibrous type with age and the proportion of fatty type were similar to usual women, but they were different from HT group. Conclusion : HT delays the degeneration of mammary gland to a certain extent, and reduce the risk of malignant lesions relatively.
