摘要
目的:探讨B-flow超声显像技术对乳腺混浊性囊肿的诊断价值。方法:选取本院2012年1月-2015年1月收治的胸外科乳腺混浊性囊肿患者90例,共120个乳腺混浊性囊肿为研究对象,全部患者均行常规高频高分辨率二维超声、彩色多普勒血流显像(CDFI)与B-flow超声显像,分析三种检查方法对乳腺混浊性囊肿的诊断效能。结果:高频高分辨率二维超声显示,有115个肿块呈椭圆形,肿块包膜完整,边界清,纵横比<1,内部低回声;另有3个肿块为类圆形混合回声,有包膜。CDFI显示上述118个肿块均未见血流信号,另2个肿块周围血流信号丰富,肿块中央区未见血流信号。B-flow超声显像显示所有肿块中液性肿块102个,实性肿块18个。病理组织学结果证实以上120个肿块中乳腺囊肿12个,纤维囊性乳腺病95个,纤维腺瘤7个,乳腺脓肿合并液化4个,导管内乳头状癌2个,B-flow超声显像对液性乳腺混浊性囊肿的诊断灵敏度、特异性、阳性预测值、阴性预测值、准确度分别为92.59%、83.33%、98.04%、55.56%、91.67%。B-flow超声显像对乳腺混浊性囊肿血流显示率明显高于高频高分辨率二维超声与CDFI,比较差异均有统计学意义(P<0.05)。结论:B-flow超声显像技术在乳腺混浊性囊肿的血流显示率更高,有助于提高诊断灵敏度、特异性,值得临床推广应用。
Objective:To discuss the diagnosis value of B-flow ultrasound imaging technology on haziness breast cyst.Method:Ninety patients with,a total of 120 haziness breast cyst in our hospital from January 2012 to January 2015 were selected as the objects,all the patients were given conventional high frequency high resolutiontwo-dimensional ultrasound,color doppler flow imaging(CDFI) and B-flow ultrasound imaging,the diagnosis value of the three examinations on haziness breast cyst were analyzed.Result:The high frequency high resolution two-dimensional ultrasound showed 115 pieces of mass oval with complete tumor capsular,clear borders,aspect ratio1,and low internal echo;3 pieces of circular mixed echo with envelope.The CDFI showed 118 pieces above had no blood flow signals,other 2 pieces with rich blood flow signal around it,but no blood flow signals in the center of the mass.The B-flow ultrasound imaging showed 102 pieces of liquid mass,18 pieces of solid mass.While the histologic results confirmed the 120 pieces above as 12 pieces of galactoma,95 pieces of fibrocystic mastopathy,7 pieces of fibroadenoma,4 pieces of breast abscess combined liquefaction,and 2 pieces of intraductal papillary carcinoma.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of B-flow ultrasound imaging on the diagnose of liquid haziness breast cyst were respectivehy 92.59%,83.33%,98.04%,55.56% and 91.67%.The blood flow detection rate of B-flow ultrasound imaging was significantly higher than the high frequency high resolution two-dimensional ultrasound and the CDFI,the differences were statistically significant(P〈0.05).Conclusion:B-flow ultrasound imaging technology has higher blood flow detection rate on the haziness breast cyst,it helps to improve diagnostic sensitivity and specificity,which is worthy of clinical popularization and application.
