摘要
目的探讨血清铁、铁蛋白、转铁蛋白、可溶性转铁蛋白受体(sTFR)与冠心病的相关性。方法选择因胸痛症状入院行冠状动脉造影检查证实为冠心病的患者21 9例,按血管累及支数分为女性单支病变组(35例)、双支病变组(35例)、3支病变组(35例);男性单支病变组(38例)、双支病变组(38例)、3支病变组(38例);另选无冠心痛患者70例,分为男性对照组35例和女性对照组35例。对不同病变组患者进行铁4项指标及血脂检测。结果与对照组比较,男性、女性不同病变组血清铁、铁蛋白、转铁蛋白和sTFR明显升高(P<0.05,P<0.01);与单支病变组比较,双支病变组、3支病变组血清铁、铁蛋白和转铁蛋白明显升高(P<0.01);男性、女性(除铁蛋白外)铁4项指标与TC、LDL、载脂蛋白B显著相关(P<0.05)。结论男性和女性铁负荷增高与冠心病的发生、发展密切相关;铁超负荷程度越大,冠状动脉病变程度越严重;铁超负荷可能是冠心病的又一新的危险因素。
Objective To investigate the relationship of serum iron（SI）,ferroprotein（SF）,transfer- rin（TRF）, soluble transferrin receptor （sTFR） with coronary heart disease （CHD）. Methods Four indexes of iron and blood lipid were detected by Roche MODULAR P-800 analyzer in 219 patients with coronary heart disease（38 male single-branch group, 38 double-branch group, 38 three-branch group and 35 female single-branch group, 35 double-branch group, 35 three-branch group） and control group（35 males and 35 females）. Results The levels of four indexes of iron were significantly higher in both male and female patients than in controls （P~ 0.05,P~ 0.01）. Compared with single-branch group, the iron indexes in double-branch group and three-branch group were significantly increased except sTFR. Four serum iron indexes in both male and female three disease groups except SF in females had significant correlation with TC, LDL and apoB. Conclusions Iron load increase has close relationship with the occurrence and development of coronary heart disease in both male and female. The greater the degree of iron overload,the more severe the coronary disease. Iron overload is another new risk factor for coronary heart disease.
