摘要
目的探讨血清EB病毒衣壳抗原IgA(EBV-VCA-IgA)检测对鼻咽癌的临床价值。方法使用酶联免疫吸附(ELISA)方法对共计1 335例健康人群、鼻咽癌、脑胶质瘤、胃癌、结直肠癌患者血清EBV-VCA-IgA)进行检测,结果采用SPSS13.0软件进行统计分析。结果鼻咽癌患者血清EBV-VCA-IgA阳性率为64.9%,健康人群为12.8%、脑胶质瘤为13.2%、胃癌为9.7%、结直肠癌为9.3%,鼻咽癌患者血清EBV-VCA-IgA阳性率较健康人群、脑胶质瘤、胃癌、结直肠癌患者明显升高(P=0.000);男性鼻咽癌患者阳性率为67.6%,女性为58.3%,但差异尚无显著性(P=0.074);随着鼻咽癌患者年龄增大,血清EBV-VCA-IgA阳性率有增高趋势,但差异无显著性(P=0.517);治疗后鼻咽癌患者血清EBV-VCA-IgA较治疗前阳性率有显著下降(P=0.018),滴度也明显下降。结论血清EBV-VCA-IgA抗体检测可以作为鼻咽癌筛查和诊断的辅助方法 ,并可以作为监测鼻咽癌疗效的潜在观察指标。
【Objective】To investigate the clinical significance of serum EBV-VCA-IgA examination to NPC.【Methods】The serum EBV-VCA-IgA was assessed by ELISA in 1335 cases including healthy adults,patients with nasopharyngeal carcinoma（NPC）,neuroglioma,gastric carcinoma and colorectal cancer.The results were statistically analyzed using SPSS13.0 software package.【Results】 The positive rates of serum EBV-VCA-IgA in NPC,healthy cases,neuroglioma,gastric carcinoma and colorectal cancer were 64.9 %,12.8 %,13.2 %,9.7 % and 9.3 % respectively.Patients with NPC had significantly higher EBV infection than those in control group and other groups（P =0.000）.Positive rate of male patients was 64.9 % and that in female patients was 58.3 % in NPC without significant difference（P =0.074）.The positive rate of serum EBV-VCA-IgA was rising with the increase of age in NPC without significant difference（P =0.517）.The positive rate and titers of serum EBV-VCA-IgA in post-treatment patients decreased obviously compared with pre-treatment patients with NPC（P =0.018）.【Conclusion】The test of serum E-BV-VCA-IgA can be used as an auxiliary method for screening and diagnosis of NPC.EBV-VCA-IgA may be a potential marker for monitoring therapeutic effect on NPC.
