摘要
目的 检测脆性组氨酸三联体(FHIT)和P14ARF蛋白在子宫内膜腺癌、子宫内膜不典型增生、正常子宫内膜组织中的表达,探讨FHIT、P14ARF基因在子宫内膜癌发生、发展过程中的可能作用及相关性.方法 采用免疫组化S-P法检测60例子宫内膜腺癌、30例子宫内膜不典型增生、30例正常子宫内膜组织中FHIT、P14ARF蛋自的表达,结合临床病理特征进行分析,并探讨FHIT与P14ARF关联性.结果 FHIT在子宫内膜腺癌、子宫内膜不典型增生和正常子宫内膜中的阳性表达率分别为53.3%、63.3%、100%,子宫内膜腺癌、子宫内膜不典增增生FHIT的阳性表达率显著低于正常子宫内膜,差异有统计学意义(P<0.01).P14ARF在子宫内膜腺癌、子宫内膜不典型增生和正常子宫内膜的阳性表达率分别为56.7%、73.3%、90.0%,子宫内膜腺癌P14ARF的阳性表达率显著低于正常子宫内膜组织,差异有统计学意义(P<0.01).FHIT的阳性表达与子宫内膜腺癌的手术-病理分期、组织分级、肌层浸润和淋巴转移相关(P<0.05),P14ARF的阳性表达仅与子宫内膜腺癌的组织分级相关(P<0.01).FHIT与P14ARF蛋白之间的阳性表达在子宫内膜腺癌中存在明显正相关性(r=0.396,P<0.01).结论 FHIT、P14ARF蛋白在子宫内膜腺癌、子宫内膜不典型增生和正常子宫内膜中的表达逐渐升高.在子宫内膜腺癌中,随着细胞分化程度由高到低,两者的表达逐渐减少,随着子宫内膜腺癌手术-病理分期的提高、子宫肌层浸润的加深和淋巴结的转移,FHIT表达逐渐减少,两个指标之间有明显的正相关性.两个指标的联合检测,对子宫内膜腺癌的早期诊断和判断预后具有一定参考价值.
Objective To detect the expression of the FHIT and P14ARF protein in endometrial adenocarcinoma, endometrial atypical hyperplasia and the normal endometrium. To examine the relation of FHIT and P14ARF in the genesis and progression of endometrial carcinoma. Methods The expression of FHIT and P14ARF protein in 60 cases of endometrial adenocarcinoma. 30 cases of endometrial atypical hyperplasia and 30 cases of normal endometrial tissue were detected by SP immunohistochemical staining. Results The positive rates of FHIT in endometrial adenocarcinoma,endometrial atypical hyperplasia and normal endometrium were 53.3%, 63.3%, 100% respectively. The positive rates of FHIT in endometrial adenocarcinoma and endometrial atypical hyperplasia was significantly lower than that in normal endometrium,The difference was statistically significant (P 〈0.01 ). The positive rates of P14ARF in endometrial adenocarcinoma,endometrial atypical hyperplasia and normal endometrium were 56.7%, 73.3%, 90.0% respectively. The positive rate of P14ARF in endometrial adenocarcinoma was significantly lower than that in normal endometrium ( P 〈0.01 ). The positive expression of FHIT was associated with the surgical-pathologic staging of endometrial adenocarcinoma, organizational grade, myometrial invasion and lymph node metastasis (P 〈 0.05 ). The positive expressions of P14ARF were only related to the organizational grade of endometrial adenocarcinoma (P 〈0.01 ). The positive expressions between FHIT and P14AR protein were of significant positive correlation in endometrial adenocarcinoma (rs = 0.396, P 〈 0.01 ). Conclusions The expression of FHIT and P14ARF protein in endometrial adenocarcinoma,endometrial atypical hyperplasia and normal endometrium are gradually increased. In endometrial adenocarcinoma, the expression of FHIT are gradually decreased with surgical-pathologic stage, myometrial invasion and lymph node metastasis, they were of significant positive correlation between the two protein.
