慢性丙型肝炎患者病毒载量与T细胞免疫功能关系分析 被引量:4

摘要 目的观察慢性丙肝病毒(HCV)感染者细胞免疫功能的变化,探讨病毒复制程度与细胞免疫功能的关系。方法应用流式细胞仪直接免疫荧光法检测120例丙肝病毒感染者和40例正常对照者外周血T细胞亚群百分率,荧光定量PCR法检测丙肝感染者血清HCV-RNA。结果 (1)HCV感染者外周血CD4+T细胞百分率及CD4+/CD8+比值分别为(35.8±9.7)%,1.6±0.7,明显低于正常对照组的(40.1±10.1)%,1.8±0.8(P<0.05,P<0.01);CD8+T细胞百分率(26.8±7.5)%明显高于正常对照组的(23.8±6.7)%(P<0.01),而CD3+T细胞百分率差异无显著性(P>0.05)。(2)低拷贝组(103~104copies/mL)和高拷贝组(105~108copies/mL)的CD4+T细胞百分率及CD4+/CD8+T比值分别为(37.1±9.9)%、(35.9±8.8)%,1.6±0.6、1.2±0.7明显低于对照组(<103copies/mL)的(40.1±10.1)%、1.9±0.8(P<0.05,P<0.01);高拷贝组和低拷贝组的CD8+T细胞百分率(29.6±8.9)%、(28.8±7.3)%,明显高于对照组的(23.8±6.7)%和RNA阴性组的(24.7±5.9)%(P<0.05),且高拷贝组的CD4+/CD8+比值明显低于低拷贝组和RNA阴性组(P<0.01);而RNA阴性组CD4+、CD8+细胞百分率与正常对照组之间及CD4+、CD8+细胞百分率和CD4+/CD8+比值在低拷贝组与高拷贝组之间无明显差异(P>0.05)。结论慢性丙型肝炎患者HCV-RNA表达水平的升高,可能是导致T淋巴细胞应答能力下降的重要原因。
