环孢素A血药浓度C_0和C_2的监测对移植肾功能的影响 被引量:2

摘要 目的探讨肾移植术后服用环孢素A(CsA)的药代动力学特性与术后早期移植免疫排斥反应及CsA肾中毒的关系。方法采用免疫荧光偏振TDX法,测定184例肾移植术后受者全血CsA浓度,其中82例监测CsA谷值浓度(C0组),102例监测CsA峰值浓度(C2组)。计算各自的药代动力学参数,回顾性分析两组CsA浓度测定值与肾移植功能状况及相关临床资料的关系。结果两组出现急性排斥反应的发生率,C0组明显高于C2组,两组差异存在显著性(P<0.05),但两组在药物性肝、肾中毒及肺感染的发生率方面比较均差异未见显著性(P>0.05)。结论通过C2进行CsA的药代动力学监测,可以较准确反映肾移植受者的药物剂量,尤其在监控急性排斥反应方面意义较大,同时,也可减少CsA肾中毒的危险。 【Objective】 To explore the clinical efficacy and safety concentration of CsA in kidney transplant recipients.【Methods】 The CsA concentrations in 184 cases were measured by fluorescence polarization immunoassay(TDX) method:82 cases were monitored for C0 level(served as group C0),102 cases were monitored for C2 level(served as group C2).The clinical data including the incidence rate of renal function after transplant,acute rejection,liver and kidney intoxication of CsA,and pneumonia were retrospectively analyzed.【Results】 The rate of acute rejection in group C2 was obviously lower than that in group C0(P 0.05),and the rates of liver and kidney intoxication of CsA and pneumonia were of no significant deviation between group C2 and group C0(P 0.05).【Conclusion】Monitoring CsA pharmacokinetics by C2 concentration can accurately reflect the drug dose in kidney transplant recipients,and is of especial significance in monitoring acute rejection.Meanwhile it can reduce renal toxicity of CsA.
