血管紧张素转换酶抑制剂及血管紧张素Ⅱ受体阻滞剂对高血压左室肥厚患者血清瘦素和一氧化氮的影响 被引量:11

摘要 目的探讨血管紧张素转换酶抑制剂贝那普利、血管紧张素Ⅱ受体阻滞剂缬沙坦对高血压并左心室肥厚(LVH)患者血清瘦素(Leptin)、一氧化氮(NO)浓度的影响。方法选取58例高血压并LVH患者(观察组),随机分为两组(缬沙坦组、贝那普利组),两组分别给予24周的缬沙坦、贝那普利干预;选取58例高血压无LVH患者为对照组。对照组及观察组治疗前后采用放射免疫法测定血清Leptin水平,硝酸还原酶法测定NO浓度。结果观察组治疗前血清Leptin水平较对照组高,NO浓度较对照组低,差异均有统计学意义(P<0.05)。高血压并LVH患者的左室质量指数(LVMI)与Leptin呈正相关,LVMI与NO呈负相关(r分别为0.276和-0.268,P均<0.05)。缬沙坦组、贝那普利组患者治疗前血清Leptin、NO及LVMI比较差异均无统计学意义(P>0.05);经24周治疗后贝那普利组较缬沙坦组血清NO浓度升高及LVMI升高,差异均有统计学意义(P<0.05)。结论 Leptin、NO分别是促进和抑制LVH的重要物质;贝那普利在逆转LVH方面优于缬沙坦。 Objective To study the effect of ACE inhibitors, angiotensin Ⅱ antagonist on serum leptin and nitric oxide (NO) in patients with hypertensive left ventricular hypertyoph. Methods 58 hypertension cases with LVH (treatment group) were randomly divided into two groups (valsartan group and benazepril group). The two groups were treated with valsartan and benazepril for 24 weeks, and meanwhile 58 hypertension cases without LVH were chosen as control group. The level of Leptin was detected by radioimmunoassay and the viscosity of NO was detected by nitrate reduetase approach before and after the treatment. Results Before treatment, the level of Leptin was higher, and the level of NO was lower compared with the control group. The difference was significant ( P 〈 0. 05 ). In hypertension cases with LVH, LVMI was positively correlated with the level of Leptin, but negatively correlated with NO ( r = 0. 276, r = - 0. 268, all P 〈 0. 05 ). The difference between the level of Leptin, the level of NO and LVMI was not significant before treatment (P 〉 0. 05 ). After 24 weeks treatment, NO level in benazepril group was higher and LVMI level was lower than that of valsartan group, the difference were all significant (P 〈 0. 05 ). Conclusion Leptin is one of the key substances which are promoting factors on LVH, but NO is one of the key substances which are inhibiting factors on LVH. Benazepril is better than valsartan on LVH.
