窄谱中波紫外线对寻常型银屑病外周血T淋巴细胞亚群的影响 被引量:7

摘要 目的观察窄谱中波紫外线(UVB)对寻常型银屑病外周血T淋巴细胞及其亚群的影响,探讨其可能的作用机制,为临床应用窄谱UVB提供更深入的理论依据。方法单独采用窄谱UVB照射治疗寻常型银屑病20例,隔日1次,共20次,在光疗前、光疗10次、20次分别采用双色荧光抗体染色技术经流式细胞仪检测外周血T淋巴细胞亚群。结果光疗前外周血CD4+及CD4+/CD8+比值均低于正常参考组(P<0.01),CD8+高于正常参考值(P<0.01),光疗过程中随着皮损的改善,银屑病及皮损面积和严重指数(PASI)积分下降,皮损T细胞亚群表现为一致性下降,光疗10次时与光疗前相比差异有统计学意义(P<0.01),光疗20次时外周血T细胞亚群接近正常水平。结论窄谱UVB照射可减少皮损中活性T淋巴细胞的浸润,减轻表皮的炎症反应,最终使外周血T细胞亚群接近正常水平,间接起到调节银屑病患者全身免疫状态的作用。 Objective To explore the mechanism and provide some guidance for the phototherapy of psoriasis with NB-UVB.Methods A total of 20 patients were only treated with 311 nm NB-UVB radiation which was performed one time every other day,totally for 20 times.T cell subsets in peripheral blood were determined by flow cytometry.Results The level of CD4^+ and CD4^+/CD8^+ in peripheral blood in patients were lower and CD8+were higher than that in normal,respectively(P〈0.01).All of them obviously decreased after treatment with NB-UVB,and PASI scores also decreased.Conclusion Radiation of NB-UVB can decrease T leukomonocyte cells in peripheral blood of psoriasis and it maytake effect through the cell immunity.
