摘要
目的观察长期服用辅酶Q10(CoQ10)对慢性心力衰竭(简称心衰)患者心房颤动(简称房颤)发生的影响。方法 128例慢性心衰患者,随机分对照组和CoQ10组。CoQ10组给予正规治疗同时给予辅酶Q10口服。随访12个月,检测血清肿瘤坏死因子(TNF)-a、白介素(IL)-6和丙二醛(MDA)的水平及左室射血分数(LVEF)、左室舒张末期内径(LVEDD),并观察房颤的发生。结果 102例完成12个月随访。共15例记录房颤事件,其中对照组12例,CoQ10组3例(P(0.05)。对照组随访6个月后,血清IL-6和TNF-a水平明显降低,血清MDA水平无明显变化;CoQ10组随访6个月后,血清IL-6、TNF-a和MDA水平均明显降低(3.4±1.1 ng/ml vs 2.3±0.9 ng/ml,3.5±0.9 ng/ml vs 1.8±0.8 ng/ml;3.9±0.7 ng/ml vs 2.5±0.6 ng/ml,P(0.05);随访12个月时,血清IL-6、TNF-a与MDA水平均再无明显下降;两组患者在随访6,12个月后,LVEF均明显升高,LVEDD明显减小。结论 CoQ10长期应用能降低慢性心衰患者房颤的发生,这种作用可能与降低MDA水平有关。
Objective To observe the effects of Coenzyme Q10 (CoQ10) on the occurrence of atrial fibrillation (AF) in the patients with chronic heart failure (CHF). Methods One hundred and twenty-eight consecutive patients with CHF were randomly divided into CoQ10 group (combined administration of common drugs and CoQ10) and control group (ad- ministration of common drugs). Ambulatory ECG Hoher monitoring (24 h), Doppler echocardiography and inflammatory cytokines were performed before treatment, six months and 12 months after treatment. Results One hundred and two patients were completed in this study. Three patients in the CoQ10 group and 12 patients in the control group had episodes of AF after 12 months treatment. The incidence of AF were lower in the CoQ10 group than in the control group (6.3% vs 22. 2% ; P 〈 0.05 ). After 6 months treatment, the level of IL-6 and TNF-a were decreased in the control group, however, there was no significant difference in the levels of malondialdehyde (MDA). There were significant reductions in the level of IL-6, TNF-a and MDA in the CoQI0 group after 6 months treatment. Furthermore, there was a significant increase in left ventricular ejection fraction and a significant reduction in the left ventricular end diastolic dimension in the two groups. Conclusion CoQ10 has benefit in reducing the incidence of AF in patients with CHF. The mechanisms of this effect may have relation with the reduced levels of MDA.
