铜绿假单胞菌耐药性与16S rRNA甲基化酶表达分析 被引量:3

Drug Resistance and Expression of 16S rRNA Methylase from Pseudomonas aeruginosa

摘要 目的了解医院铜绿假单胞菌(PAE)中16S rRNA甲基化酶表达情况及与耐药关系。方法用K-B纸片法进行药敏试验,采用聚合酶链反应(PCR)扩增68株铜绿假单胞菌16S rRNA甲基化酶armA、rmtA、rmtB、rmtC、rmtD和npmA基因。结果 68株铜绿假单胞菌对阿米卡星、庆大霉素、头孢他啶及磺胺甲噁唑/甲氧苄啶的耐药率分别为51.5%、67.6%、63.2%和78.0%,呈高水平耐药,而对链霉素仍具有一定的敏感性,armA、rmtB和rmtC的检出率分别为5.9%、29.4%及2.9%,未检出rmtA、rmtD和npmA基因,其中16S rRNA甲基化酶阳性株对阿米卡星和庆大霉素的耐药达到100.0%。结论铜绿假单胞菌多药耐药可能与16S rRNA甲基化酶表达情况有关,在PAE 16S rRNA甲基化酶中检出rmtC基因,加强对耐药菌株的检测,严格按药敏结果用药及通过交替换药等方式有助于减少或延缓PAE耐药菌株的传播与流行。 OBJECTIVE To investigate the expression of 16S rRNA methylase genes and its relation with the drug resistance of Pseudomonas aeruginosa. METHODS The drug susceptibility was tested by K-B method and 16Sr RNA methylase genes (armA, rmtA, rmtB, rmtC, rmtD and npmA) were detected by polymerase chain reaction (PCR). RESULTS The resistance rates of 68 strains of PAE to amikacin were 51. 5%, gentamiein 67.6%, cefazolin 63. 2% and snlfamethoxazole/trimethoprim 78. 0%, respectively, but they had certain sensitivity to streptomycin. The detection rates of armA, rmtB and rmtC were 5. 9%, 29. 4% and 2. 9%, respectively, but rmtA. rmtD and npmA genes were not detected out in 68 strains of PAE. The resistant rates of positive strains of 16S rRNA methylase to amikacin and gentamicin were up to 100%. CONCLUSION Multidrug resistance of PAE is possibly related to the expression status of 16S rRNA methylase, rmtC has been detected from16S rRNA methylase in PAE. To strengthen the detection of drug-resistant strains, and to apply drug in accordance with the drug susceptibility and alternate application of drugs can help to reduce or postpone the spread and prevalence of PAE drug-resistant strains.
