极度耐药铜绿假单胞菌与鲍氏不动杆菌对多黏菌素B的MIC值分布调查

Distribution of MIC value of Polymyxin B against Extensively Drug-resistant Pseudomonas aeruginosa and Acinetobacter baumannii

摘要 目的检测多黏菌素B体外对极度耐药(XDR)铜绿假单胞菌与鲍氏不动杆菌的最低抑菌浓度(MIC),评价其对该类细菌的抗菌活性,为临床治疗、控制感染提供依据。方法临床分离的288株细菌(铜绿假单胞菌130株,鲍氏不动杆菌158株),采用纸片扩散法检测临床常见的抗菌药物,以确定极度耐药菌;用E-试验进行多黏菌素B及临床常用11种抗菌药物MIC值的检测,分析菌株的MIC值分布情况。结果 288株菌均为XDR,对临床常用的11种抗菌药物的敏感性均<15%;对多黏菌素B的MIC值均≤2g/ml;130株铜绿假单胞菌与158株鲍氏不动杆菌对多黏菌素B的MIC50均为0.5g/ml,MIC90均为1.0g/ml。结论多黏菌素B体外对极度耐药铜绿假单胞菌与鲍氏不动杆菌均敏感,未发现有多黏菌素B耐药株;但存在敏感区域内的高MIC值菌株,应引起临床重视。 OBJECTIVE To study the in vitro activity of Polymyxin B against Extensive drug-resistant Pseudomonas aeruginosa and Acinetobacter baumannii clinical isolates by detecting MIC,to provide reference for the clinical treatment and infection control.METHODS The extensive drug-resistant bacteria were confirmed by using KB method for these isolates on 25 kinds of antibiotics.The antimicrobial susceptibility to 11 kinds of clinical routine antimicrobial agents and MIC of Polymyxin B in 130 strains of extensive drug-resistant P.aeruginosa and 158 strains of Extensive drug-resistant A.baumannii by E-test method.RESULTS A total of 288 strains were confirmed that these isolates were extensive drug-resistant bacteria.For 130 strains of extensive drug-resistant P.aeruginosa and 158 strains of extensive drug-resistant A.baumannii,the antimicrobial susceptibility of all of the 11 kinds of clinical routine antimicrobial agents were lower 15%.For 130 strains Extensive drug-resistant P.aeruginosa and 158 strains of extensive drug-resistant A.baumannii,the MIC50 and MIC90 to Polymyxin B were both 0.5 g/ml and 1.0 g/ml.CONCLUSION In vitro Polymyxin B are sensitive to both Extensive drug-resistant P.aeruginosa and A.baumannii.No Polymyxin B-resistant strains are found out in this study,but there are strains with high MIC value in sensitive region.
