摘要
目的比较全数字化钼靶(FFDM)与MRI动态增强(MRI-DCE)检查在乳腺肿瘤良、恶性鉴别中的临床价值。方法回顾性分析经病理证实的37例乳腺肿瘤,其中恶性22例,良性15例,所有病例均行FFDM和MRI-DCE检查,在GEADW4.3工作站进行后处理,观察X线表现、动态增强表现,绘制时间-信号强度曲线(time-intensity curves,TIC),分别计算良、恶性肿瘤的早期强化率,根据美国放射学会乳腺影像报告和数据系统(breast imaging reporting and data system,BI-RADS)进行定级,3级及以下的级别考虑为良性,4A级及以上的为恶性,与病理结果对照,分别计算各自的敏感性、特异性、准确性。结果 FFDM和MRI-DCE的敏感性、特异性、准确性各依次为80.00%、79.17%、79.48%;88.23%、90.91%、89.74%。两者特异性有显著性差异(P<0.05),两者确诊的病例部分互为补充。结论在FFDM的基础上,选择性的对可疑病变行MRI-DCE检查,可以提高良、恶性鉴别诊断的准确性,从而降低手术活检的几率,提高患者的生存质量。
Objective To contrast the value of mammography and Dynamic Enhanced Magnetic Resonance Imaging in diagnosing breast diseases,especially between benign and malignant identification.Methods Breast X-ray and MRI-DCE were taken in 37 patients with breat tumors,including 24 malignant lesions and 13 benign lesions histologically confirmed.To observe the X-ray and DCE performance,draw time-intensity curves,contrast two kinds of mammography diagnosis and pathological results,then calculate the early signal intensity enhancement rate.Based on American College of Radiology breast imaging reporting and data system(breast imaging reporting and data system,BI-RADS),calculate separately of sensitivity,specificity and accuracy of benign and malignant diagnoses.Results of sensitivity,specificity and accuracy of breast benign and malignant differential diagnoses are 80.00%,79.17%,79.48% and 88.23%,90.91%,89.74%.The difference of specificity was significant(P0.05).Conclusion To perform the MRI-DCE alternatively on the base of FFDM can improve the accuracy of the suspicious lesions.It can decrease incidence of biopsy and raise the patient′s living quality.
