摘要
目的初步探讨红景天用于防治鼻咽癌同步放化疗近期毒副反应的临床效果。方法收集广州医学院附属肿瘤医院放疗科26例局部中晚期鼻咽癌患者的临床资料,年龄24~65岁,中位年龄43.0岁。26例患者随机分成研究组14例和对照组12例,两组患者均行同期放化疗:鼻咽剂量68~80Gy,颈区剂量60~70Gy,每次2.0~2.19Gy,每周5次;同期化疗采用以铂类为基础方案。研究组口服红景天胶囊,每次500mg,每天3次,自同步放化疗第1天起直至放疗结束。采用NCICTC3.0版毒性标准评价患者近期毒性情况。结果研究组坚持口服红景天时间平均为3.4w。研究组与对照组相比,粘膜毒性、放射性皮炎、脱发、食欲减退、恶心、呕吐、便秘、疲乏、失眠、血细胞下降及肝、肾、心脏等近期毒性指标均未见统计学差异(p>0.05);然而,坚持红景天口服超过5w患者与对照组相比,疲劳毒性减轻(p<0.05)。结论红景天可能有助于改善鼻咽癌同步放化疗期间的疲劳反应。鼻咽癌同步放化疗期间采用口服给药依从性较差,希望后续研究加予注意。
Objective Study preliminarily about the clinical effect of Rhodiola rosea on the prevention and cure of recent toxicity from radiochemotherapy of nasopharyngeal carcinoma(NPC).Methods We Collected 26 patient with middle and advanced NPC(without distant metastasis).Their ages were from 24 to 65 years old.26 patients were randomized to either study or control group(14 and 12 patients respectively).All 26 patients received radiochemotherapy:Dose of nasopharynx and neck node were respectively 68~80 Gy and 60~70 Gy,2.0~2.19 Gy/F,5F/week;and chemical scheme were on the base of platinum.Patients in study group took orally Rhodiola rosea capsule,500mg at one time and three times one day,from the beginning to the end of radiation.Results Mean time of patients in study group taking orally Rhodiola rosea capsule was 3.4 weeks.Compared with control group,patients in study group had not statistic difference in all the indicators of recent toxicity include radio-mucositis、radio-dermatitis、defluvium capillorum、anorexia、nausea、vomit、astriction、fatigue、hyposomnia、decrease of hemocyte、and the toxity of liver/kidney/heart(p〉0.05);However,the patients that had took orally Rhodiola rosea more than 5 weeks had less fatigue toxicity than the patients in control group.Conclusion Rhodiola rosea is perhaps able to relieve the fatigue toxicity during the radiochemotherapy of NPC.It has little compliance taking orally drug during the radiochemotherapy of NPC,which is a lesson for more later study.
