摘要
目的探讨急性一氧化碳(CO)中毒后迟发性脑病(DEACMP)患者治疗前后血及脑脊液(CSF)中乙酰胆碱(Ach)水平变化及其意义。方法采用Hestrin碱性羟胺比色法检测45例DEACMP患者治疗前后血和CSF中Ach水平,用日常生活能力量表(ADL)、常识-记忆-注意测验(IMCT)、长谷川痴呆量表(HDS)评价病情,并与40例其他脑病患者和38例非脑病患者进行比较。结果 DEACMP组治疗前血Ach水平[(638.02±126.57)×10-6mol/L]明显低于非脑病组[(737.16±175.51)×10-6mol/L]及其他脑病组[(765.92±203.63)×10-6mol/L](均P<0.01);治疗后血Ach水平[(692.88±180.07)×10-6mol/L]升高,但与治疗前相比差异无统计学意义。DEACMP组治疗前CSFAch水平[(634.69±175.33)×10-6mol/L]明显高于非脑病组[(412.93±94.48)×10-6mol/L]及其他脑病组[(408.66±127.72)×10-6mol/L](均P<0.01),治疗后[(414.63±108.46)×10-6mol/L]显著下降(P<0.01)。DEACMP组治疗前后ADL、IMCT、HDS评分差异有统计学意义(均P<0.01)。结论 DEACMP患者治疗前后血和CSFAch水平变化与病情变化基本一致,动态检测Ach水平变化可作为DEACMP病情变化和治疗效果的生物学指标之一。
Objective To explore the changes and its significance of acetylcholine(Ach) content in blood and cerebrospinal fluid (CSF) in patients with delayed encephalopathy after acute carbon monoxide poisoning (DEACMP).Methods The Ach contents in blood and CSF were detected by means of Hestrin alkaline hydroxylamine colorimetric in the 45 patients with DEACMP;the activity of daily living scale(ADL),information-memory-concentration test (IMCT) and Hasegawa's dementia scale (HDS) were used to appraise the pathogenetic condition.The results were compared with 40 patients with other-encephalopathy and 38 patients with non-encephalopathy.Results The blood Ach content in DEACMP group before treatment [(638.02±126.57)×10-6mol/L] was significantly lower than that in non-encephalopathy group [(737.16±175.51)×10-6mol/L] and other-encephalopathy group [(765.92±203.63) ×10-6mol/L] (all P〈0.01);after treatment,which was increased [(692.88±180.07) ×10-6mol/L],but was no significantly different than before treatment.The CSF Ach content [(634.69±175.33)×10-6mol/L] in DEACMP group before treatment was significantly higher than that in non-encephalopathy group [(412.93±94.48)×10-6mol/L] and other-encephalopathy group [(408.66±127.72)×10-6mol/L] (all P〈0.01);after treatment,which was significantly decreased [(414.63±108.46)×10-6mol/L] (P〈0.01).There were statistical significance compared the scores of ADL,IMCT and HDS before and after treatment in DEACMP group (all P〈0.01).Conclusion The dynamic changes of blood and CSF Ach contents in DEACMP patients are basically consistent with the patient's pathogenetic condition and can be used as one of biological indicators to reflect the condition changes and treatment effects in DEACMP patients.
