摘要
目的:探讨WB-DWI+MR/CT的联合应用对骨转移瘤的临床应用价值。方法:55名健康志愿者行WB-DWI+常规MR,35例恶性肿瘤患者均在1周内接受WB-DWI、常规MR及CT检查。部分病例后续行MR增强扫描。所有骨转移瘤判断结果均经手术病理、MR/CT检查或者临床随访5≥个月(病灶范围扩大、数目增多、病变处症状加重)证实。结果:55名健康志愿者中轴骨骨髓ADC值为(0.471±0.114)×10-3mm2/s。35例恶性肿瘤患者中23例骨转移瘤患者骨转移区的ADC值为(1.611±0.564)×10-3mm2/s。正常骨髓与骨转移瘤的骨髓比较差异有统计学意义,P<0.05;依据WB-DWI+MR/CT诊断骨转移瘤的灵敏度为97.22%,特异度为53.66%,假阴性率为2.77%,假阳性率为46.34%,准确度为82.55%;依据CT/MR诊断骨转移瘤的灵敏度为70.37%,特异度为53.66%,假阴性率为29.63%,假阳性率为46.34%,准确度为65.77%。因此,依据WB-DWI+MR/CT综合影像比依据MR/CT影像诊断骨转移瘤差异有统计学意义,P<0.05。结论:WB-DWI+MR/CT方法的联合应用是检查骨转移瘤的一种快速、高效而又实用的技术,比单独应用MR/CT具有较高的灵敏度和准确度,具有显著的临床应用价值。
OBJECTIVE:To investigate the values of whole-body diffusion weighted imaging with background suppression(WB-DWI) and common MR/CT in diagnosing the whole-body bone metastasis.METHODS:Fifty healthy volunteers and thirty-five patients with bone metastasis were enrolled in this study,healthy volunteers underwent routine MR and DWI,and many healthy volunteers underwent WB-DWI+ MR/CT,35 patients with malignant metastasis underwent WB-DWI,MR,and CT in one week.RESULTS:The ADC values of fifty healthy volunteers were (0.471±0.114)×10^-3 mm^2/s.The ADC values of twenty-three bone metastasis about thirty-five patients with metastasis metastasis were (1.611±0.564)×10^-3 mm^2/s.There were significant usefulness between the normal bone marrow and bone metastasis (P〈0.05);The sensitivity,specifity,false negative rate,false positive rate,and accuracy for detecting the bone metastasis were 97.22%,53.66%,2.77%,46.34% and 82.55% respectively by routine MR/CT in combination with WB-DWI;and 70.37%,53.66%,29.63%,46.34% and 65.77% respectively by routine CT/MR.So,there was significant difference in diagnosing bone metastasis between these two methods (P〈0.05).CONCLUSION:WB-DWI+MR/CT is a rapid,efficient and practical method in detecting bone metastasis tumor and has much more sensitivity and accuracy and a significant value in clinical application comparing with MR/CT.
