CT脑灌注成像对早期脑梗死缺血半暗带诊断的初步研究 被引量:10

摘要 目的:探讨CT脑灌注成像下缺血半暗带(IP)的血流动力学参数变化规律,总结其影像表现特点。材料与方法:35例发病时间1-24h的急性脑梗死患者行常规CT、脑灌注成像(CTPI)和4-14天复查CT确定IP的范围,定量分析IP区脑血流量(CBF),血容量(CBV),对比剂平均通过时间(MTT),并与梗死中心区及对侧镜像区灌注参数对比。结果:35例发病时间<6h的患者CTPI显示IP区,10例发病时间6-24h的患者CTPI像显示IP区,半暗带脑血流量,达峰时间与对侧差异有显著性意义。(P<0.01),而脑血容量差异没有显著性意义(P<0.05)。CTPI图像灰阶呈深浅阶梯样分布特点。结论:CT脑灌注成像能够灵敏判断IP的存在,为临床对早期脑梗死患者的选择治疗方案及预后评估提供重要依据。
