肾上腺髓质素基因多态性与糖尿病肾病易感性的研究

摘要 目的探讨肾上腺髓质素(AM)基因微卫星多态性与中国汉族人群糖尿病肾病之间的关系。方法采用微卫星多态性(STR)结合聚合酶链反应(PCR)技术,比较正常中国汉族人群(健康对照组,n=77)和2型糖尿病(n=94)伴(微量白蛋白尿组和大量白蛋白尿组)或不伴肾病(正常白蛋白尿组)患者之间AM基因微卫星多态性等位基因频率和基因型频率之间的差异。结果①健康对照组、正常白蛋白尿组、微量白蛋白尿组和大量白蛋白尿组间AM基因1~4型频率分布44例(28.6%)、72例(46.7%)、37例(24.0%)、1例(0.6%)比28例(25.5%)、49例(44.5%)、32例(29.1%)、1例(0.9%)比18例(28.1%)、32例(50.0%)、11例(17.2%)、3例(4.7%)比1例(7.1%)、7例(50.0%)、2例(14.2%)、4例(28.6%),差异有统计学意义(χ2=61.900,P<0.01);AM等位基因频率分布差异亦有统计学意义(χ2=68.926,P<0.01);②正常白蛋白尿组和异常白蛋白尿组AM基因型频率分布差异无统计学意义(χ2=13.892,P=0.085)。AM等位基因频率分布差异有统计学意义(χ2=9.276,P=0.026)。AM等位基因4与异常白蛋白尿的发生相关联(RR=7.660,P<0.05),差异具有统计学意义。结论此微卫星多态位点与江苏地区汉族糖尿病肾病遗传易感性相关联。 Objective To explore the relationship between microsatellite DNA polymorphism of human adrenomedullin gene and diabetic nephropathy within a Chinese population.Methods Among Han Chinese in Jiangsu province,94 patients with type 2 diabetes mellitus and 77 normal control were collected randomly.Polymorphism of microsatellite markers was detected by performing polymerase chain reaction,polyacrylamide gel electrophoresis and silver staining,to compare the alleles frequencies and genotype frequencies of AM gene microsatellite(CA)n between diabetic patients with and without nephropathy.Results ①The genotype frequencies and alleles frequencies of AM gene microsatellite(CA)n among the normal control,diabetic patients with normal and abnormal albuminuria were significantly different 44 cases(28.6%),72 cases(46.7%),37 cases(24.0%),1 case(0.6%) vs 28 cases(25.5%),49 cases(44.5%),32 cases(29.1%),1 case(0.9%) vs 18 cases(28.1%),32 cases(50.0%),11 cases(17.2%),3 cases(4.7%) vs 1 case(7.1%),7 cases(50.0%),2 cases(14.2%),4 cases(28.6%)(χ2=61.900,P〈0.01;χ2=68.926,P〈0.01).②The alleles frequencies of AM gene microsatellite(CA)n between normal and abnormal albuminuria in diabetic patients were significantly different(χ2=9.276,P=0.026),whereas no significant difference was found in the genotype frequencies.The allele-4 was more common in diabetic patients with abnormal albuminuria(RR=7.660,P〈0.05).Conclusion Polymorphism of microsatellite markers in AM gene is associated with diabetic nephropathy in Han Chinese.
