摘要
目的 观察喉罩置入对婴幼儿呼吸循环的生理影响。方法 3 0例 ( 1~ 3 )岁、ASA( ~ )级的患儿随机分成两组 ,分别采用喉罩 ( n=15 ,L 组 )和气管插管 ( n=15 ,T组 )维持通气。分别于置入前 1、2、3分 ,置入即刻 ,置入后 1、2、3、4、5分记录患儿 Ppeak、DP、SP、PETCO2 等数值 ,并描计 P/VL oop图表。结果 两组置入前 Ppeak、DP、SP、PETCO2 无明显差别 ,置入即刻与置入后 1、2分 ,T组SP、DP、Ppeak均明显高于 L 组 ,而 L 组 PETCO2 明显高于 T组。结论 喉罩置入较气管插管反应轻微 ,气道阻力小 ,但鉴于 PETCO2 明显升高 ,建议婴幼儿使用喉罩进行辅助呼吸时 ,宜进行气体监测 ,以免 CO2
Objective To observe the influence of the insertion of laryngeal mask (LM) on the pediatric cardiopulmonary system Methods Thirty patients ASA grade Ⅰ Ⅱ,aging 1 3 years, were randomly divided into two groups:laryngeal mask group(Group L, n=15) and endotracheal intubation group(Group T,n=15) P ET CO 2,Peak pressure and arterial pressure were measured 1,2,3 min before insertion or at insertive moment and post insertive 1,2,3,4 and 5 min Results There were no significant difference in pre insertive SP,DP,Ppeak,P ET CO 2 between both groups ,SP,DP,Ppeak were significantly higher in Group T at post insertive 0,1,2 min ,however P ET CO 2 significantly lower than in group L at postinsertion Conclusion In comparison with endotracheal intubation,LM insertion to pediatric patient can induce less stress responses,be ventilated more easily,but be liable to develop the retained CO 2,so the LM application requires the monitoring of P ET CO 2
