摘要
目的探讨临床医学与公共卫生结合的管理模式对儿童喘息的效果。方法以248例5岁以下儿童喘息作为研究对象,分为观察组和对照组。对照组以最新儿童喘息诊治方案治疗,并配合以普通的健康教育;观察组在此基础上,增加健康教育家庭化。纳入管理半年后,统计两组患儿的治疗效果,以首诊和管理半年后情况作对比,包括:喘息严重程度、年平均发作天数、年急诊次数。同时,对比观察组和对照组情况,以评价健康教育家庭化的治疗效果。纳入管理半年后,两组患儿家属还接受调查,项目包括:哮喘常识的掌握情况、公卫及预防常识的掌握情况、自我监控的能力,改变不良行为习惯的发生情况,并由临床医生和健康医生评价其规范用药、定期复诊等遵医行为,以评价健康教育家庭化对儿童喘息管理的效果。结果所有考核指标(包括管理指标和疗效评价),观察组与对照组比较,差异均有统计学意义。结论健康教育家庭化利于儿童喘息的管理和治疗。临床医学与公共卫生结合的管理模式值得探讨和推广。
Objective To explore the effect of the combined management mode with clinical medicine and public health in managing children with wheezing.Methods 248 cases of wheezing children under 5-year-old were selected and divided into the observation group and the control group.The control group received the latest diagnosis and treatment program,and the ordinary health education;while the observation group received additional family-oriented health education based on the ordinary health education.The curative effects,including the severity of wheezing,the average number of days with attack in a year,the average number of days with emergency in a year,were evaluated after six months of management.The effects of family-oriented health education was also compared between the both groups.The survey on asthma common-sense understanding,public health and prevention common-sense understanding,the ability of self-monitoring,the ability of changing in the occurrence of bad behavior were performed in family members of the two groups.The therapeutic effect on wheezing children of family-oriented health education treatment was appraised through evaluating the standard medication,compliance behaviors such as return visit on a regular basis by clinicians and health physicians.Results All the assessment indicators,including management indicators and response assessment,had statistical difference between the observation group and control group.Conclusion Family-oriented health education is conducive to the management and treatment of children with wheezing,and combination of clinical and public health management model should be explored and promoted.
