摘要
目的 观察普罗布考对急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)后炎性因子和血管内皮功能的影响.方法 78例AMI患者均行冠状动脉造影及PCI治疗,术后分为对照组40例和普罗布考组38例.两组均常规使用硝酸酯类和他汀类药物,普罗布考组在常规用药基础上加服普罗布考500 mg/次,每日2次.记录所有患者出院后6个月内心血管事件,检测血脂、炎症相关生物标志物和血管内皮功能指标.结果 出院后6个月内普罗布考组心血管事件发生率显著低于对照组(P〈0.05);6个月后普罗布考组血脂、血清白介素-6(IL-6)、高敏C反应蛋白(hs-CRP)、血内皮素-1(ET-1)及氧化低密度脂蛋白(ox-LDL)比对照组显著降低(P〈0.01或P〈0.05);普罗布考组血一氧化氮(N0)及内皮依赖性血管舒张功能(FMD)比治疗前显著升高(P〈0.05).对照组血NO、ET-1、ox-LDL及FMD治疗前后差异无统计学意义(P〉0.05).普罗布考组未发现严重的不良反应.结论 普罗布考可减轻AMI患者PCI术后炎性反应,改善血管内皮功能,且不良反应少.
Objective To observe the effect of Probucol on the function of vascular endothelial and inflammatory response in patients with acute myocardial infarction after intervention. Methods 78 patients with AMI patients underwent coronary anglography and PCI treatment,78 patients were divided into control group(40 patients) and the Probucol group (38 patients) after PCI treatment. Two groups of patients were routine use nitrates, statins. Probucol group plus clothing Probucol on the basis of conventional medication, each dose of 500mg, 2 times a day. Cardiovascular events of all patients after discharge from hospital within 6 months were recorded. The serum lipids,inflammation-related biomarkers and indicators of vascular endothelial function were measured. Results The cardiovascular events in Probucol group were significantly lower than that of the control group after discharge within 6 months (P〈0.05). After 6 months treatment, the serum lipids, IL-6, hs-CRP, plasma ET-1 and ox-LDL in Probucol group were significantly lower than that of the control group (P〈0.01 or P〈 0.05). The serum NO and FMD in Probucol group were significantly higher than before treatment (P〈0.05), the serum NO, plasma ET-1, ox-LDL and FMD in the control group were no significant difference before and after treatment (P〉0.05). No serious adverse events were found in Probucol group. Conclusion Probucol reduces inflammatory reaction after PCI in patients with AMI,improves endothelial function, and has less adverse reactions.
