摘要
目的研究对比典型支气管哮喘(bronchial asthma,BA)与咳嗽变异性哮喘(cough-variant asthma,CVA)最大气道反应性的差异及其反应平台的分布情况。方法选择2007年3月~2009年2月我院呼吸科住院或门诊就诊的BA和CVA患者,其中BA组46例、CVA组42例,分别吸入较高浓度的醋甲胆碱作为支气管激发剂,当第1秒用力呼气容积(FEV1)下降至激发前水平的50%或出现最大反应平台后终止试验,比较两组出现反应平台的比例及平台水平值。结果 BA组16例出现反应平台(34.8%),CVA组24例出现反应平台(57.1%),两组比较差异有统计学意义(P〈0.05);BA组反应平台的水平值为(36.74±6.02)%,CVA组为(32.85±6.52)%,两组比较差异有统计学意义(P〈0.05);BA组与CVA组FEV1下降20%时的醋甲胆碱含量(PC20)分别为(2.60±3.04)mg/ml、(2.93±3.32)mg/ml,差异无统计学意义(P〉0.05)。结论 BA组气道反应性显著高于CVA组,表明气道高反应性与其临床表现和病情严重程度相关。
Objective To investigate the difference of maximal airway response and compare the prevalence of response plateau between patients with bronchial asthma(BA)and cough-variant asthma(CVA).Methods There were 46 patients in BA group and 42 patients in CVA group.High-dose methacholine inhalation test was carried out in patients in BA group and CVA group respectively.The procedure was terminated when the forced expiratory volume in one second(FEV1)fell below 50% of pre-provocation test,while maximal response plateau was established.Finally,we compared the proportion of response plateau and the levels between the two groups(P0.05).Results 16(34.8%)patients in BA group and 24(57.1%)in CVA group showed maximal response plateau with significant difference between the two groups.Patients in BA and CVA groups with response plateau showed significantly different plateau levels(BA 36.74±6.02 % vs.CVA 32.85±6.52 %,P0.05).However,the provocative concentration of methacholine causing a 20% fall in FEV1(PC20)between the two groups(2.60±3.04)mg/ml vs.(2.93±3.32)mg/ml did not show difference of statistical significance(P0.05).Conclusion Maximal airway response may demonstrate the correlation between the airway hypersensitivity,clinical expression and severity of the disease.Furthermore,classic asthma has higher airway responsiveness than cough-variant asthma.
