摘要
目的探讨微量元素铜、锌、钙、铁、铅、镉异常对儿童骨密度(BMD)的影响。方法将686名健康儿童按年龄段分为4组。Ⅰ组,幼儿期;Ⅱ组,学龄前期;Ⅲ组,学龄期;Ⅳ组,青春期。所有儿童均进行BMD的检测和微量元素的检测,根据BMD检测结果,分成BMD正常组与减低组,比较两组微量元素异常的发病率、各年龄组BMD减低的发生率及BMD减低组中各年龄段血清微量元素的差异。结果①BMD减低组锌、钙、铁缺乏的发病率明显高于正常组,差异有统计学意义;铜缺乏,铅、镉中毒的发病率两组比较差异无统计学意义。②各年龄段BMD减低的发生率比较,差异有统计学意义,其中Ⅰ组BMD减低的发生率最高,达45.7%,且BMD减低的发生率随年龄增长有逐渐减低的趋势。③BMD减低组中,平均血清锌水平Ⅰ组明显低于其他三组、Ⅱ组明显低于Ⅳ组,差异有统计学意义;平均血清钙水平Ⅰ组明显低于Ⅲ组、Ⅳ组,Ⅱ组明显低于Ⅳ组,差异有统计学意义;平均血清铁水平Ⅰ组明显低于Ⅲ组、Ⅳ组,Ⅱ组、Ⅲ组明显低于Ⅳ组,差异有统计学意义;平均血铅水平Ⅰ组、Ⅱ组明显高于Ⅳ组,差异有统计学意义;血清铜、镉两种元素含量各组比较差异无统计学意义。结论血清锌、钙、铁、铅四种微量元素异常可能会引起儿童骨密度降低。
Objective To explore the effects of abnormal levels of trace elements such as copper,zinc,calcium,iron,lead and cadmium on children's bone mineral density(BMD).Methods Total 686 healthy children were divided into 4 groups by ages,infant(groupⅠ),pre-school(groupⅡ),school age(group Ⅲ),adolescence(group Ⅳ).Test of BMD and the serum level of six trace elements were conducted in all children.They were assigned to the BMD normal group and the BMD lower group according to the result of BMD test.The differences of the morbidity of abnormal levels of trace elements were compared between the two groups.The differences of the incidence of low BMD were compared be-tween different ages.The differences of the serum levels of the trace elements were compared between different ages in the lower group.Results The morbidity of the zinc,calcium and iron insufficiency in the lower BMD group were higher than those in the normal group with significant difference.The difference of the morbidity of copper insufficiency and increased lead and cadmium levels were not significant between the two groups.The difference of the morbidity of lower BMD was significant between different ages.Among different ages,the morbidity of lower BMD was the highest in groupⅠ,which reached to 45.7%,and the morbidity of lower BMD was tended to gradually decrease with age.In the lower BMD group,the average serum zinc level in group Ⅰ was lower than that in the other groups,and that in group Ⅱ was lower than in groups Ⅳ,both showing significant differences.The average levels of serum calcium in group Ⅰ was lower than that in group Ⅲ and Ⅳ,and that in groups Ⅱ was lower than group Ⅳ,both with significant differences.The average level of serum iron in groupⅠwas lower than that in groups Ⅲ and Ⅳ,and groups Ⅱand Ⅲ was lower than group Ⅳ,both showing significant difference.The average levels of serum lead in groups Ⅰ and Ⅱ was higher than that in group Ⅳ,with significant difference.The average levels
