摘要
目的观察高血压合并糖耐量减低(IGT)者颈动脉粥样硬化的特点,探讨胰岛素抵抗在高血压病动脉粥样硬化中的作用。方法本科住院或门诊患者72例,随机分为两组:单纯高血压组(A组)34例;高血压合并IGT组(B组)38例;健康体检者36例作为对照组(C组)。所有患者均行颈部动脉超声检查。测定空腹血糖和胰岛素,计算稳态模式评估法的胰岛素抵抗指数(HOMA-IR)。结果与C组比较,A组颈总动脉内膜中层厚度(IMT)、颈动脉分叉处IMT、Crouse积分显著增高;B组颈总动脉IMT、Crouse积分显著高于A组(P<0.01)。结论高血压合并IGT患者颈动脉粥样硬化程度显著高于单纯高血压者,IGT和高血压同时存在时可加速动脉粥样硬化的进展,其机制可能与胰岛素抵抗有关。
Objective To investigate the characteristics of carotid artery atherosclerosis in essential hypertensive patients with impaired glucose tolerance(IGT),and evaluate the role of insulin resistance in the pathogenesis of this disease.Methods 72 patients were enrolled into this study,and were divided into three groups:essential hypertensive group(A group,n=34),essential hypertension with impaired glucose tolerance group(B group,n=38),and healthy control group(C group,n=36).All these subjects were detected with carotid Doppler ultrasonography.The homeostasis model assessment insulin resistance index(HOMA-IR)was calcucated based upon the levels of fasting plasma glucose(FPG)and fasting insulin(FINS).Results The intima-media thickness(IMT)of common carotid artery,bifurcate IMT of carotid artery and carotid arterial plaque score were higher in patients with hypertension than those in control group.Patients in B group demonstrated the highest levels of parameters above-mentioned among these three groups.Conclusion In essential hypertensive patients with IGT,the extent of atherosclerosis is worse than those with hypertension alone.Insulin resistance may play an important role in the pathogenesis of atherosclerosis.
