摘要
目的观察益肾平肝方对阴虚阳亢型高血压病人动态血压及血管内皮功能的影响。方法收集高血压早期肾损害患者90例,随机分为3组,中药组(益肾平肝方),中西药结合组(益肾平肝方+洛汀新),西药组(洛汀新),用药3个月,观测治疗前后的24 h动态血压、血清内皮素(ET)及一氧化氮(NO)的变化。结果中药组治疗后动态血压各指标与治疗前比较均有显著下降(P<0.01或P<0.05);3组组间比较各指标无显著差异;中药组治疗前后ET、NO两指标有显著差异(P<0.01),中药组、中西药结合组与西药组组间比较差异显著(P<0.01)。结论益肾平肝方可有效降低原发性高血压患者的白昼和夜间血压,并在一定程度上可改善和逆转异常的血压昼夜节律;该药的降压作用可能是通过影响血管内皮功能实现的。
Objective To observe the effects of Yishen Pinggan Decoction on ambulatory blood pressure and vascular endothelial function of Yinxu-Yangkang hypertensive patients.Methods 90 cases of hypertensive patients with early renal damage were randomly divided into 3 groups,Chinese medicine group（Yishen Pinggan Decoction）,Chinese medicine plus western group（Yishen Pinggan Decoction ＋ Lotensin）,taking western medicine group（Lotensin） medicine for three months,and 24h ambulatory blood pressure,serum ET and NO were observed before and after treatment.Results After treatment,the each indicators of ambulatory blood pressure of Chinese medicine group were significantly decreased compared with those before treatment（P〈0.01 of P〈0.05）.There were no significant difference among three sets of inter-group comparison;Before and after treatment,the levels of ET and NO were different significantly in Chinese medicine group（P〈0.01）.There was significant difference armong Chinese medicine group and western effect and Chinese medicine group combined with western medicine group（P〈0.01）.Conclusion Yishen Pinggan Decoction is effective in patients with essential hypertension to reduce the daytime and nighttime blood pressure,and improve and reverse the abnormal circadian rhythm of blood pressure to some extent.Hypotensive effect of the decoction may due to the regulation for vascular endothelial function.
