ΔNp63和VEGF在膀胱移行细胞癌中的表达及临床意义

摘要 目的探讨ΔNp63和VEGF在膀胱移行上皮癌(TCCB)中的免疫组化表达及与膀胱癌发生发展中可能的作用和相互关系。方法用免疫组化染色方法对56例膀胱移行上皮癌组织行ΔNp63和VEGF检测,将结果与病理分级、分期和预后进行分析。结果膀胱移行细胞癌中ΔNp63和VEGF阳性表达率明显高于正常膀胱粘膜(P<0.05)。ΔNp63和VEGF在低分化、浸润性癌组织中的阳性表达率明显高于高分化、浅表性癌组织,在复发病例中的阳性表达率显著高于初发病例,2个标记物在膀胱癌的病理分级、分期和预后之间表达差异有统计学意义(P<0.05)。采用Spearman等级相关性分析,ΔNp63与VEGF呈正相关。结论ΔNp63在膀胱癌中的异常表达是膀胱癌组织向浸润性发展和恶性程度增加的重要原因之一,可能通过促进肿瘤血管生成发挥促癌作用,ΔNp63可能是判断TCCB预后的生物学指标。 Objective To investigate the immunohistochemistry expressions of △Np63 and VEGF in transitional cell carcinoma of bladder (TCCB) and relationship between the expressions of △Np63, VEGF and the pathological stage, grade and prognosis of bladder cancer. Methods The expressions of △Np63 and VEGF on pathological sections of 56 patients with TCCB were detected with immunohistochemical staining. The immunoassay results were analyzed in relation to the tumor stage, grade and the clinical prognosis. Results The expressions of TCC-positive rate of △Np63 and VEGF were higher than that in normal bladder mucous membrane(P〈0. 05). The results show that the expressions of TCC- positive rate of △Np63 and VEGF in the poorly differentiated and invasive TCCB were significantly higher than the well differentiated and superficial TCCB. The expressions of TCC-positive rate of △Np63 and VEGF in the recurred cases was much higher than the primary cases(P〈0. 05). The expression difference of the two markers between the pathology grade, clinical stage and prognosis had statistical significance(P〈0.05). Conclusions Abnormal expressions of △Np63 is one of the significant factor of the increasing of the degree of the invasion and malignancy of the bladder cancer, and possibly play a role in tumor promotion by promoting tumor angiogenesis. △Np63 may be a biological index to determine the prognosis of TCCB.
