摘要
目的评价透视下复位单边外固定支架外固定与经皮三枚空心加压钛螺钉内固定的微创方法治疗闭合性损伤老年性股骨粗隆间骨折的疗效。方法对我院2004年6月~2008年3月通过上述两种方法治疗的63例老年性股骨粗隆间骨折病例进行回顾性分析。按AO原则分类,将股骨粗隆间骨折分为A1、A2、A3三种类型,根据不同类型闭合性损伤老年性股骨粗隆间骨折分别采用三枚空心加压钛螺钉内固定方法和单边外固定支架外固定方法进行治疗。A1型骨折均使用空心钛钉,A2型骨折、A3型骨折使用外固定支架外固定固定疗法。结果63例病人术后均获随访,随访时间13~59月,平均21.6月,按黄公怡临床评定标准,优39例,良19例,可5例,优良率:92%。结论根据此类高年龄患者,全身情况差、并发多种内科疾病、手术风险高、患者本人不愿意承受创伤大的手术痛苦特点,选择手术创伤小,手术时间短,风险低,固定较为可靠,经济适用,患者利于接受的方法。C型臂X线机透视下闭合复位三枚空心加压钛螺钉内固定与外固定支架外固定是一种常用简单有效的治疗方法。
Objective To estimate the efficacy of the treatment of closed femoral intertrochanteric fractures in elderly patients with unilateral external fixation apparatus or three parallel compression screws under X-ray fluoroscopy.Methods Follow-up survey on 63 elderly patients with closed femoral intertrochanteric fractures was analysised retro- spective, who were treated with unilateral external fixation apparatus or three parallel compression screws from June 2004 to March 2008. According to AO classification, patients of type A1 were treated with unilateral external fixation apparatus, patients of type A2 and A3 were treated with three parallel compression screws. Results All the 63 patients were followed up for 13-59 months, averaged 21.6 months. The outcome was evaluated according to Huang Gongyi's criteria, 39 cases had the excellent resule, 19 cases good, 5 cases fair. The excellent and good rate was 92%. Conclusions The aged people with the femoral intertroehanteric fractures usually accompanied with several internal medical disorders, and their general conditions were poor and the tolerance of operation was not good. Hower, the treatment method of closed intertrochanteric femoral fractures in elderly patients with unilateral external fixation apparatus or three parallel compression screws under X-ray fluoroscopy was simple and effective with short time, of firm fixation, mininal injury, less bleeding and with high ratio of bone healing.
