摘要
目的:探讨原发性肝癌(primary hepatic carcinoma,PHC)的血流灌注特点。方法:对94例PHC及29例无肝脏疾病患者(对照组)进行肝脏CT灌注扫描,采用斜率法计算其肝动脉灌注量(hepatic arterial perfusion,HAP)、门静脉灌注量(hepatic portal perfusion,HPP)、总肝灌注量(total liver perfusion,TLP)、肝动脉灌注指数(hepatic arterial perfusion index,HAPI)和门静脉灌注指数(hepatic portal perfuiosn index,HPPI),对比分析PHC和对照组血流灌注参数差异。结果:PHC组织HAP和HAPI显著高于对照组(p<0.0001),HPP、TLP和HPPI显著低于对照组(p<0.0001)。结论:CT灌注成像可以精确测定PHC血流灌注参数;PHC表现为高动脉灌注和低门静脉灌注。
Objective: To study the hemodynamic characteristics of primary hepatic carcinomas(PHC).Methods: Multidetector computed tomography perfusion imaging was performed in 94 patients with PHC and 29 control subjects without liver diseases.Hepatic arterial perfusion(HAP),hepatic portal perfusion(HPP),total liver perfusion(TLP),hepatic arterial perfusion index(HAPI) and hepatic portal perfusion index(HPPI) were calculated by the maximum slope method.Results:The HAP and HAPI values in patients with PHC were significantly higher than those in the control group(P〈0.0001),and HPP,TLP and HPPI values in patients with PHC were markedly lower than those in the control group respectively(P〈0.0001).Conclusions :Perfusion CT can provide quantitative information about arterial and portal perfusion of liver tumors.Primary hepatic carcinomas demonstrated their hemodynamics as high arterial perfusion and low portal perfusion.
