摘要
目的观察氟比洛芬酯减轻神经外科手术患者拔管期不良反应及其用于自控镇痛的临床效果。方法择期神经外科手术患者30例,随机分成氟比洛芬酯组(F组)和对照组(C组),两组分别于术毕前1h给予氟比洛芬酯注射液100m(g10ml)和生理盐水10ml,拔管后分别给予氟比洛芬酯复合芬太尼及单纯芬太尼自控镇痛。观察记录两组患者拔管时间、拔管期生命体征变化及拔管后1h、12h、24h疼痛程度评分(VAS)、镇静程度评分(SS)及不良反应。结果拔管期:F组平均动脉压、心率及1级以上躁动发生率显著低于C组(P<0.05);拔管后镇痛:F组VAS评分和C组比较无统计学差异,SS及不良反应发生率显著低于C组(P<0.05或P<0.01)。结论氟比洛芬酯用于神经外科手术患者拔管期可以减轻气管拔管的不良反应;复合芬太尼用于患者自控镇痛可以减少芬太尼引起的不良反应,提高镇痛质量。
Objectives To observe the clinical effect of flurbiprofen axetil on the side reactions during tracheal extubation and postoperative self-controlled analgesia in the patients undergoing neurosurgical procedures. Methods Thirty neurosurgical patients were randomly divided into two groups, i.e. F and C groups, of 15 patients each. Flurbiprofen axetil lOOmg (in F group) or 10ml normal saline (in C group) was intravenously injected about 1 hour before the finishing of the operations. Flurbiprofen axetil and fentanyl (in F group) or only fentanyl (in C group) was giverl to the patients after the tracheal extubation for self-controlled analgesia. During the period of tracheal extubation, vital sign, extubation time (from stopping infusion of anaesthetics to extubation) and the side reactions were observed. The visual analogue scale (VAS), sedation score (SS), and the side reaction were evaluated during postoperative self-controlled analgesia.Results During the period of tracheal extubation, mean arterial pressure, heart rate and the side reactions were significantly lower in F group than those in C group (/~〈0.05). During postoperative self-controlled analgesia, there was insignificant difference in VAS between the two groups (P〉0.05), but the SS and rate of the side reactions occurrence in F group were significantly lower than those in C group (P〈 0.05). Conclusion Flurbiprofen axetil can reduce the side reactions during the tracheal extubation in the patients undergoing neurosurgical procedures, and can reduce theside effects of fentanyl and offers overall favorable analgesic quality when it and fentanyl are used together during postoperative self-controlled analgesia.
