摘要
目的了解某三甲医院4年间的医院内真菌感染状况。方法采用查阅临床病例资料,回顾性分析了从2005年1月-2008年12月医院内真菌感染情况。结果4年医院内真菌感染的平均发病率为8.04%;感染部位以下呼吸道最多,占55.07%,其次为泌尿道和皮肤;感染因素以抗菌药物的应用最多,占42.03%,其次为呼吸机使用,占17.87%;病原菌种类以白色假丝酵母菌最多(93.72%),207例真菌感染培养中,只有64例(30.92%)做过药敏试验,各菌株对5-氟胞嘧啶最为敏感,对伊曲康唑耐药性最强;所有真菌感染病例均使用过抗菌药物,两种抗菌药物联用占21.26%,3种抗菌药物联用占9.66%。结论以呼吸道医院内真菌感染最多见;最主要的病原菌为白色假丝酵母菌,并存在忽视病原学检查及不合理使用抗菌药物,因此应加强管理,以减少医院内真菌感染的发生。
OBJECTIVE To investigate the nosocomial fungal infection from Jan 2005 to Dec 2008 in same a hospitaland analyze its pathogens,infection routin,high-risk factors,drug-resistance and relationships with the use of antibiotics.METHODS The clinical data of patients with fungal nosocomial infection from Jan 2005 to Dec 2008 were retrospectively analyzed.Pathogens were identified by fungal biochemical identification panel,antimicrobial susceptibility tests were done by disc agar diffusion test.RESULTS The mean incidence rate of nosocomial fungal infections in a hospital from 2005 to 2008 was 8.04%.Most infection site was lower respiratory tract infection,accounted for 54.5%,followed by urinary tract and skin infection.Among high-risk factors,antibiotics use accounted for 42%,followed by ventilator use,accounted for 17.9%.Candida albicans is the major type of pathogens,accounted for 93.7%.Drug sensitivity tests were carried out in 64 cases（30.9%）,in which the strains were most sensitive to 5-FC,showing strongest resistance to Itraconazole.The percent of antibacterial usage was 100.0% in the patients with nosocomial fungal infection,21.3% used with two antibacterials,and 9.66% used with three ones.CONCLUSIONS Lower respiratory tract is the most frequent infection site.The main pathogen of nosocomial fungal infection is C.albicans;the management including rational use of antibiotics,microbiogical culture and susceptibility test should be strengthened to reduce the nosocomial fungal infection.
