三维超声在胎儿神经系统畸形诊断中的应用价值 被引量:3

摘要 目的评价三维超声在胎儿神经系统畸形诊断中的应用价值。方法应用PHILIPS HD I4000、PHILIPS iu 22三维超声对42789例孕18~40周胎儿进行常规二维和实时三维超声检查。结果42789例胎儿中共检出胎儿神经系统畸形157例,其中,脑积水31例(合并脊柱裂20例),露脑畸形15例,无脑儿22例,脑膨出4例,Dandy-Walker综合征3例,全前脑10例,脑裂2例,胼胝体发育不全18例,蛛网膜囊肿7例,室管膜囊肿6例,脉络膜囊肿38例,颅内出血1例。结论三维超声成像较二维超声能提供更丰富的诊断信息,直观显示胎儿颅内结构,提高了胎儿神经系统畸形的检出率,对指导临床及时终止妊娠和保证出生人口质量具有重要意义。 Objective To assess the applied value of real-time three dimensional ultrasonography(3DUS) for diagnosing fetal nervous system malformations (FNSM). Methods 42,789 fetuses with gestional age of 16-40 week previously underwent conventional two dimensional ultrasonography (2DUS) to understand fetal direction and subsequently underwent real-time 3DUS. All ultrasonic examinations were performed by using PHILIPS HD1400 and PHILIPS iu 22 color multifunction ultrasonograph. Results Among 42,789 fetuses, real-time 3DUS showed nervous system malformations in 157 fetuses who comprised 31 hydrocephalus (20 of whom associated with spina bifida), 15 exencephaly, 22 anencephaly,4 encephalocele,3 Dandy-Walker syndrome,10 holoprosencephaly, 2 schizencephaly, 18 callosal agenesis ,7 arachniod cyst,6 ependymal cyst,38 choroidal cyst, and 1 intracranial hemorrhage. Conclusion Real-time 3DUS may provide more informations than 2DUS in detecting FNSM and may directly show the intracranial structures of the fetuses, simultaneously, it elevated the detective rate of FNSM and therefore it has important significance for timely terminate the pregnancy and precisely ensure the birth quality of population.
