摘要
目的探讨不同胸交感神经链切断后手温变化规律,寻找最佳手术范围。方法气管插管全身麻醉,30°～45°半坐位,两臂外展90°,电视纵隔镜从腋前线第3肋间小切口置入,电钩切断胸交感神经链。将体温监测器贴在手掌心,对比术前术后手掌温度。结果手术时间20～40 min,平均30 min。单纯手汗合并而部及腋窝多汗均消失,双手温度上升0.8～3℃,温暖而干燥。住院3～7 d,平均3.5 d。无严重并发症发生。结论治疗手汗症最佳手术范围是切断胸3或胸4交感神经。
[ Objective ] To investigate the rule of variation in hand temperature after resection of different chest sympathetic chains under video-mediastinoscope, and to search for the best operation range. [ Methods ] Tracheal intubation and general anesthesia were done, patients took 30°- 45°semi-sitting position with both arms abducting 90°. Mediastinoscope was inserted in a small incisal opening in the 3rd intercostal space, chest sympathetic chains were cut down with electro-hook. Temperature monitor was put in the middle palm of hand and the preoperative and post-operative temperatures were compared. [ Results ] Operation time was about 20-40 minutes, the average time was 30 minutes. Both simple hand sweating and hand with axillary sweating disappeared. And both hands of the patients were warm and dry, and their temperature elevated 0.8-3℃. The time of hospital stay was 3-7 days, the average was 3.54 days. There was no serious complication. [ Conclusion ] The best operation range for palmar hyperhidrosis is to cut down the third or fourth chest svmoathetic nerve.
