摘要
目的 探讨腺病毒介导的白细胞介素10(Ad-IL-10)基因转染对呼吸机相关性肺损伤大鼠全身炎性反应的影响。方法健康成年雄性SD大鼠30只,体重200~220g,随机分为5组(n=6):空白对照组(C组)、通气2h空白腺病毒组(B1组)、通气4h空白腺病毒组(B,组)、通气2hAd—IL-10基因转染组(A1组)和通气4hAd—IL-10基因转染组(A2组)。C组不做任何处理,B.组和B1组分别于麻醉后右心室腔内注射空白腺病毒0.1ml,A,组和A2组分别于麻醉后右心室腔内注射含IL-10基因的腺病毒0.1ml,滴度1×10^9pfu/ml,48h后行机械通气,维持气道压力25cmH2O各组于通气2h或4h时放血处死大鼠,采集腹主动脉血样5ml,测定血清IL-8和IL10浓度,观察肺组织病理学结果及超微结构。结果与C组比较,其余各组血清IL-8和IL-10浓度升高(P〈0.01);与B1组比较,B2组血清IL-8和IL-10浓度升高,A1组血清IL-8浓度降低,IL-10浓度升高(P〈0.05或0.01);与A2组比较,A1组血清IL-8和IL10浓度降低,B2组血清IL-8浓度升高,IL10浓度降低(P〈0.05或0.01)。B1组和B2组病理损伤程度严重,A1组和A2组病理损伤程度减轻,A2组损伤减轻程度更明显。结论Ad-IL-10基因转染可减轻呼吸机相关性肺损伤大鼠全身炎性反应,从而减轻肺损伤。
Objective To investigate the effect of adenoviral-mediated interleukin-10 (IL-10) gene transfection on systemic inflanmatory responses in a rat model of ventilator-induced lung injury. Methods Thirty adult male SD rats weighing 200-220 g were randomly divided into 5 groups ( n = 6 each) : group Ⅰ control (group C) ; group Ⅱ adenovirus without IL-10 gene + 2 h mechanical ventilation (MV) (group B1 ) ; group Ⅲ adenovirus without IL-10 gene + 4 h MV (group B2 ) ; group IV adenovirus with IL-10 gene + 2 h MV (group A1 ) and group Ⅴadenovirus with IL-10 gene + 4 h MV (group A2 ). In group Ⅱ-V the animals were mechanically ventilated (airway pressure = 25 cm H2O) and adenovirus was injected into right ventricle at 48 h before MV. Blood samples were taken from abdominal aorta at the end of 2 h or 4 h MV for determination of serum IL-8 and IL-10 concentrations. The animals were then killed. The puhnonary specimens were obtained for examination of uhrastructure with light and electron microscope. Results Mechanical ventilation significantly increased serum IL-8 and IL-10 concentrations in a duration-dependent manner in group Ⅱ- V as compared with group C. Transpnhnonary administration of adenoviral-mediated interleukin-10 gene significantly decreased sermn IL-8 concentration and increased serum IL-10 concentration and reduced ventilator-induced lung injury after MV in group A1 and A2 as compared with group B1 and B2 .Conclusion Adenoviral-madiated IL-10 gene transfection can attenuate the systemic inflammatory response and reduce ventilator-induced lung injury.
