速度向量成像——检测正常左室壁加速度的新方法

摘要 目的:应用速度向量成像(velocity vector imaging,VVI)技术研究正常左室壁加速度(acceleration,ACC)特点。方法:应用Acuson Sequoia 512彩色多普勒成像仪携带的VVI技术,检测35例健康志愿者的左室16个节段运动速度并推演出ACC。结果:①加速度-时间曲线的形态变化较稳定,但不同人幅度可有较大波动。②ACC的分布特点:正常左室同一室壁不同时相的正向和负向ACC,均表现为自基底向心尖递减,同一水平各室壁之间舒张期AC-Cd和收缩期ACCs均无显著性差异,同一节段ACCd显著大于ACCs(P<0.05)。结论:VVI技术可检测正常左室壁ACC并分析其特点,为进一步研究病变心肌ACC的变化提供理想工具。 AIM: To explore the characteristics of segmental myocardial acceleration rate (ACC) of normal left ventricle (LV) with velocity vector imaging (VVI), a novel imaging technique used to accurately reveal velocity magnitude and direction of left ventricular myocardial movement in diastolic and systolic cycles. METHODS: VVI integrated in ultrasound imaging system from ACUSON Limited Corporation was performed on 16 segments of LV walls in 35 normal volunteers, and segmental myocardial ACC was derived from velocity. RESULTS : The shape of ACC vs. time curve was steady, but the magnitude varied greatly among individuals examined. The ACC magnitude from normal LV wall descended from base to apex and no significant differences were observed between LV walls of the same level in the early systolic ACCs and early diastolic peak ACCd. ACCd was significantly higher than ACCs in the same segment (P 〈0.05). CONCLUSION: NVVI can be used to study the acceleration rate of normal left ventricle and its characteristics, which facilitates further investigation of the acceleration rate of diseased myocardium in clinical study and provides powerful support for accurate diagnosis and proper operation schemes.
