CD4+CD25+Tregs在慢性乙型肝炎的发病机制中作用 被引量:2

摘要 慢性乙型肝炎感染者针对病毒产生免疫反应较弱,病毒在体内不能彻底清除,感染持续存在,组织损伤迁延不愈,逐渐发展为肝硬化和肝细胞癌,而目前其发病机制仍不清楚,是世界医学难题。CD4+CD25+调节性T细胞是由胸腺T细胞自然分化发育而来的有较强的免疫抑制作用的T淋巴细胞群,具有自身免疫无能和免疫抑制特性,可抑制天然及获得性免疫系统的功能,对于维持免疫耐受及稳态具有重要作用。近年研究表明,其与慢性乙型肝炎的发生机制可能相关。有研究表明机体的Tregs的数量增多且可抑制CD4+T细胞和CD8+T细胞的活化增殖及细胞因子的分泌,从而影响机体抗感染免疫应答的发生,导致感染的持续。先后列举了国外Franzese、Stoop和国内福军亮、徐东平、韩捷等研究者的关于CD4+CD25+调节性T细胞在慢性乙型肝炎的发病机制中作用的研究结果。现今研究虽然很多,但突破性的研究并不多见,人类CD4+CD25+Tregs表型鉴定标志仍不十分明确,体内外功能研究的结果并不趋于一致。但至少目前我们可以有这样一种假设,慢性乙型肝炎患者体内的CD4+CD25+Tregs数量或者功能异常,降低了机体的乙型肝炎病毒特异性的效应性T淋巴细胞反应,使得乙型肝炎病毒不能得到彻底清除,反复感染。如果这一机制得到进一步证实,可能会给慢性乙型肝炎带来新的治疗手段。
