Observation and cell phenotype detection of proliferative preretinal membranes in proliferative diabetic retinopathy

摘要 目的探讨增生性糖尿病视网膜病变(proliferative diabetic retinopathy,PDR)的视网膜增生前膜的病理形态改变和细胞表型的变化。方法视网膜前膜组织来自10例PDR患者玻璃体切割手术中取出的视网膜前增生膜,直接铺片进行光镜暗视野模式下观察形态变化。视网膜前增生膜组织包埋在冷冻胶中,冰冻切片后进行转化生长因子-β1(transforming growth factorβ1,TGF-β1)、上皮细胞钙黏蛋白和波形蛋白免疫组织化学研究。结果PDR患者的视网膜前膜内含大量毛细血管,并包含有色素上皮细胞成分,可观察到无管腔的内皮细胞条索和微血管瘤。免疫组织化学观察表明TGF-β1在8例Ⅴ期PDR患者血管增生膜中强表达,而对照组正常视网膜中表达阴性。增生膜内钙黏蛋白E表达较弱,而波形蛋白表达呈强阳性。结论PDR的视网膜前膜内含丰富血管和细胞,TGF-β1表达的升高可能是细胞表型变化的原因之一。 Objective To explore the changes of pathological morphology and cell phenotype for proliferative preretinal membranes in proliferative diabetic retinopathy(PDR).Methods The preretinal membrane tissues were collected from proliferative preretinal membranes of 10 PDR patients who were being operated with vitrectomy.The tissues were taken stretched preparation directly and morphological changes were observed at dark field mode under light microscope.The tissues were embedded in cryogen and the immunohistochemical studies of transforming growth factor β1(TGF-β1),E-cadherin and vimentin were taken after frozen section.Results There were considerable blood capillaries and pigment epithelial cell components in preretinal membranes of PDR patients,and non-lumen endothelial trabs and microangioma were also found.Immunohistochemistry showed that TGF-β1 was with a high expression in proliferative preretinal membranes of 8 PDR patients at Ⅴ stage and with negative expression in retina of control group,E-cadherin seldom expressed and vimentin with high expression.Conclusions There are multiple blood vessels and cells in preretinal membranes of PDR.The high expression of TGF-β1 may be one reason for changes of cell phenotype.
