摘要
目的观察普贝生用于足月妊娠促宫颈成熟和引产的临床效果、安全性，探讨其护理措施。方法将100例孕周37—41W无阴道试产禁忌症、单胎、头位的足月待产妇阴道内放置普贝生1枚，用药12h后，观察宫颈Bishop评分，宫缩发动时间，强直宫缩，分娩方式，新生儿及羊水情况。结果①引产结果：宫颈Bishop评分用药后12h内提高≥2分者为86例，有效率86％；宫颈评分提高≥3分者为80例，显效率80％；引产成功75例。②阴道分娩70例，占70％；剖宫产30例，占30％。③新生儿及羊水情况：新生儿无一例发生窒息，评分均为良好；羊水Ⅱ度污染4例，其余均为羊水清。结论普贝生可安全、有效、方便地用于促宫颈成熟和引产。在普贝生引产过程中应严密观察宫缩及产程进展，严格做好交接班，掌握好取药的适宜时间。
Objective To observe the effectiveness and safety when using Prepess to promote cervical maturity and induced delivery in full term pregnancies, as well as discuss the nusing. Methods 100 case of 37 - 41 w single fetal first pregnancies were observed, which vaginal delivery had been prohibited in each case due to unfavorable fetal head position. 12 hours after applying a minimal amount of Propess to the vaginal posterior fomix, evaluation of cervix using Bishop method, rigid uterine contractions, time of onset of uterine contractions, time of delivery, method of delivery, and birth conditions of new born were carried out. Results ①Induction of labor results： There were 86 cases of cervical Bishop score within 12h after the drug added ≥2, which accounted for 86% and 80 cases of cervical Bishop score added ≥3, which accounted for 80%. 75 cases of successful induction of labor. ②70 cases were vaginal delivery, accounting for 70% ; 30 cases of cesarean section, accounting for 30%. ③ The condition of newborn and amniotic fluid： None of the cases was fetal distress or neonatal asphyxia. II amniotie fluid contamination in 4 eases, all the rest of anmiotie fluid clearance. Conclusions Usage of Propeas is safe, effective and convenient for maturing cervix and can induce delivery, thereby effectively reduce cesarean section rates. Clinical evaluation of propess in the process of induction and the uterine contraction stage of labor should be closely observed. At the same time, the medicine appropriate time should be mastered well.
