体外培养骨髓间充质干细胞经大鼠冠状动脉内移植的可行性研究 被引量:2

摘要 目的:探讨冠状动脉内移植骨髓间充质干细胞(MSCs)的可行性和安全性。方法:左侧开胸结扎sD大鼠左冠状动脉建立心肌梗死模型。①离体实验:取心肌梗死后2周大鼠5只,开胸取出心脏,常规建立Langendorff模型。将CM—DiI标记的MSCs悬液注入主动脉根部,同时收集右心房回流的液体,离心后通过流式细胞仪计数细胞数量。观察是否有细胞经冠状静脉回流及数量比例,并在细胞移植前和移植后5、15和30min测左室收缩压(LVSP)、左室舒张压(LVDP)、左室压力变化率(±dp/dt)、心率,评价其安全性。②体内实验:取心肌梗死后2周的大鼠,经左心室-主动脉途径将细胞悬液注射到临时阻断的主动脉根部,分别在移植后1、24h和1、4周取材观察细胞在心脏内的位置及细胞的迁移情况。结果:离体实验发现经冠状动脉内注射MSCs(95±3)%的细胞在心肌组织内存留,移植后大鼠LVSP、LVDP、±dp/dt及心率没有明显变化(P〉0.05)。在体内实验中发现细胞移植后早期大部分细胞分布在心外膜下心肌组织内,在心内膜下组织内较少分布,而且大部分细胞在正常心肌组织内,只有少量在心肌梗死区域。移植后1~4周存活的细胞多在心肌梗死及交界区组织内,在正常组织内很少。结论:经冠状动脉内途径行MSCs移植是安全可行的。 Objective:This study was designed to investigate the retained rate,distribution and emigration of the bone marrow mesenchymal stem cells (MSCs) after transcoronary infusion and further evaluate the feasibility of BMMSCs injected into coronary artery. Methods: MSCs were isolated,purified,expanded and labelled with CM-DiI. (1)In vitro study,the infarcted SD rats'hearts were removed and perfused with Langendorff apparatus. The cells were injected into the aortic root and the fluid returning from the coronary system was collected and the labelled cells in the coronary effluent were quantitated with floweytometry. At the same time,left ventricle function (LVSP) and (LVDP) , ±dp/dt and heart rate were recorded 5,15 and 30 min before and after the transplantation to evaluate the safety of this approach. (2)In vivo study,the cells were then injected into the briefly distally clamped ascending aorta through a catheter inserted through the left ventricle into the aortic root. The hearts were harvested at different time points of 1 h,24 h and 1 week and 4 weeks after the cell transplantation to obtain the direct evidence of distribution and emigration of the implanted cells. Results:In vitro study,we found that only 3%-6% of transplanted cells returned into the right ventricle and (95 ± 3) % cells retained in the heart after injecting into the aortic root of Langendorff model of infarcted hearts of SD rats. Left ventricle function did not deteriorate after cell transplantation ( P 〉0. 05 ). In vivo study,the labelled ceils were entrapped within the coronary capillary immediately after cell infusion, mainly in the normal area. After 24 hours some cells migrated through the capillary wall into interstitium of the heart. 1 week later we found that most survival cells were located at the infarcted area and the border zone. Conclusion:MSCs transplantation via coronary artery is safe and feasible.
