摘要 目的探讨心肺转流(CPB)期间含氧血持续肺动脉灌注对心脏瓣膜置换术(CVR)患者的肺保护作用。方法30例择期CVR患者随机均分为肺灌注氧合血组(灌注组)和对照组。于CPB前、入ICU30min和拔除气管插管前行动脉血气分析,计算呼吸指数(RI)。停CPB后30min取右上肺组织,观察肺组织形态学变化。结果灌注组入ICU30min和拔除气管插管前的RI明显低于对照组(P<0.05)。灌注组肺组织病理变化较对照组小。结论肺动脉持续灌注氧合血可减轻CPB所致肺损伤。 Objective To explore the protective effects of pulmonary artery perfusion with oxygenated blood on the lungs during cardiopulmonary bypass (CPB). Methods Thirty patients undergoing selective cardiac valve replacement under CPB were randomly divided into two groups with 15 cases each. The patients in group A were given pulmonary artery perfusion with oxygenated blood continuously during CPB, and those in group B were without as the controls. Blood samples were drawn from radial artery for blood gas analysis before CPB, at 30 rain after entering ICU and before tracheal extubation,and respiratory index(RI) was calculated. Right lung tissues were obtained at 30 min after CPB for histological examination. Results At 30 min after entering ICU and before extubation, the RI of group A was lower than that of group B(P〈0.05). The histological change of the lungs after surgery was less in group A than that in group B. Conclusion Pulmonary artery perfusion with oxygenated blood during CPB can diminish CPB-induced lung injury.
