摘要
目的:探讨腹腔镜治疗小儿腹股沟斜疝及交通性鞘膜积液的手术方法。方法:用腹腔镜高位缝扎联合生物蛋白胶填塞内环口,治疗小儿腹股沟斜疝及交通性鞘膜积液120例,均于腹腔镜下行内环口荷包缝合高位结扎术、明胶海绵和纤维蛋白封闭剂填塞内环口、阴囊积液抽出术。结果:手术时间单侧5～10min,双侧10～20min。术后平均住院1.5d,皮肤切口无需拆线。无并发症发生。随访6～36个月,平均24个月,未见复发。结论:腹腔镜内环口高位结扎明胶海绵和纤维蛋白封闭剂填塞内环口,阴囊积液抽出术,具有安全有效、患者创伤小、无瘢痕、康复快、住院时间短、操作简便等优点,无需特殊手术器械,术后无阴囊感染、睾丸扭转、缺血坏死、切口感染、髂腹股沟及髂腹下神经损伤、腹股沟区疼痛麻木等并发症发生,疗效满意。
Objective ： To study a new method of laparoscopic treatment for indirect inguinal hernia in children and communicating hydrocele testis. Methods：One hundred and twenty children with indirect inguinal hernia and communicating hydrocele testis were treated by laparoscopic high ligation comhined with packing of biological fibrin glue into orifice of internal ring. All patients underwent laparoscopic purse-string suture and high ligation of orifice of inner ring, packing of gelatin sponge and fibrin glue, and drainage of scrotal hydrocele. Results ：The average operative time was 5-10min in unilateral hernia and 10-20min in bilateral hernia. The mean of postoperative hospital stay was 36h. Skin gntures were not required. All patients had been followed-up for 6-36 months （ mean 24 months）. No complications and recurrences were detected. Conclusions ： Laparoscopic high ligation of orifice of internal ring, packing of gelatin sponge and fibrin glue, and drainage of scrotal hydrocele is safe, effective, simple and satisfactory, with less trauma, quick recovery, short hospital stay, and without scar or special surgical apparatus. There are no complications such as infection of scrotum and incision, torsion, ischemia and necrosis of testis, no damage of ilioinguinal and iliohypogastric nerve, pain and numbness in inguinal area and so on.
